When do you unlock the higher Teir equipment? i can only go up to tier 4

what level are you exactly and how are you judging that you are "unlocking" equipment levels, I'm level 12 on my current playthrough, tier 6-7ish stuff is dropping for me from regular mob looting.

Don't confuse the "tiers" of Combat Training (of which there are only ever 4) with weapon and armour tiers. They don't directly relate as far as I can tell-the primary requirements on weapons and armour appear to be stats and class, not Combat Training.

Combat training unlocks the next tier of skills, not the next tier of items. Items scale based on your level.

If you can't equip items, it means your ability scores are too low. You need higher strength for most melee weapons and armor, higher dexterity for daggers and bows, and higher magic for mage staves.
