Me? Well I didn't know that I had to upgrade the Normandy so I went through Omega 4 with casualties already. Then I didn't want to risk Tali(She was my LI at the time) so I sent Mordin through the vents as my teck specialists. To my suprise the freaking "genius" didn't make it! After that I had a little more casualties. At the end I survived. I didn't have a bad amount of casualties.

Everyone survived. Gave Harbringer the finger, told the Illusive Man off, went off to vacay with the squad.

Lost Miranda and Legion. Had to choose sides in the Miranda/Jack fight and went with Jack. Unfortunately chose unloyal Miranda to lead the fire team, so lost Legion to a rocket. Miranda died on the platform after resigning from Cerberus. Everyone else + Normandy crew made it, which I thought was decent for going in spoiler free. Tried again and everyone lived (Garrus led the fire team and unloyal Miranda stayed with everyone to hold the line). Just tried killing everyone to harvest clips for a video -- it was pretty awful.

Well, the first time i went through the Omega Relay, I was sure i wouldn't lose anyone since all my squad members were loyal. All was well, until life decided to B****-slap me and killed off Zaeed when i chose him to lead the fire team. Needless to say, I was shocked (which soon turned into depression, for some reason).

I'm just going to go wallow in depression because just mentioning it makes me sad...*sigh*

I somehow managed to fail the mission by talking about 'Justicars' and 'Family', because I didn't update Thane. They seemed totally reasonable to me at the time.

Before we even got to the base, I lost Jack to a lack of armor. Why didn't I have the armor? Because I didn't want to speak to Jacob. But it was Jack who payed for it, reminding me that there is no room for pettiness when in command.

The rest of the mission went surprisingly well. No other casualties.

Lost Thane because I romanced Tali and wouldn't risk her. I had just got Legion did his LM right before the SM so I wanted to keep him around to get additonal dialog. Thane was dying anyway so I sent him and hoped for the best. He is an infiltrator type and his comments about the heat and dryness in the vents being comfortable had me thinking I might have made a good choice afterall and he might just make it. Hehe.

Nobody else died. The tasks were all mindnumbingly obvious. If not for me being cautious with certain characters and willing to sacrifice others none would have died.

I can understand it's all part of the game, you play it you deal with it. But, I can't stand being forcefully thrown into a situation without being ready. Without having completed everything prior to the point of no return. Horiz pissed me off the first playthrough. (side note) So on my first run of ME2, and It was before ME1, I done everything before the SM. Just so happened, a lot of that was after getting the go ahead from EDI. I lost the crew, expt Doc. I remember a lot of swearing at the platforms.( Imagine that.) I put Tali in the vents and Garrus on the Fire Team. I sent Thane on the escort and brought Samara along for the Barrier. I can't remember ( Wow...big shocker there ) who I had for the final fight
Everyone was loyal, although I was playing a rental and thus no Zman or Kasmi, but I may have lost Mordin. The fact that I sent Thane back, may have put my avg lower than it calls for, to save the whole squad.

My first Tali tally through the vent and take Garrus lead the first fire squad.
I sent grump back with the survivors from the Normandy and I had of the Zieed  read the second fire team.my only casualties were is Zieed and Mordin not really sure why he died though.

so I got out with only two casualties.

Everybody lived for me. I picked the most logical choices i could and they all ended up being right. Made the ending feel good as i worked on my first play through for almost a month making sure i did everything.

Kelly & the rest of the crew were melted down, didn't realize getting the IFF started a countdown and I still had loyalty missions left.  Miranda and Legion died during hold the line, I had switched from renegade to paragon decisions halfway through the game so neither score was high enough to keep everyone loyal.  Destroyed the base, and to top it all off Shepard creeped me out with some of his lines in the Tali romance (no offense to anyone who happens to like the romance with Tali or the character, Mark Meer just has a line or two in there that I really regretted picking).

lost mordin my fist run through. so i reloaded and had mordin in my party while fighting the terminator and he survived. i learned later on that mordin is the best choice to send back as the escort.mordin seems to always die while trying to "hold the line". dont know why...

My first time through went really well, I sent Tali through the vent , Miranda led the fire team, Thane escorted the recovered crew members, and Samara with the bubble, I had all the upgrades so I didn't lose anyone when I went throught the Omega 4 relay. I think it was the second or third time I went through it I lost Jack (not sure why), but she is the only one I've ever lost.

Lost half the crew (didn't actually think there was a timer; I thought that was a fixed plotpoint) and Jack because of the non-transparent paragon/renegade system.

Thanks to my friend who finished ME2 before me, I knew what I had to do. She didn't tell me what will happen in the end, only that I need to upgrade my ship before the Suicide Mission. So I did it. And all my teammates and the crew survived. The Collector Base was destroyed and the Illusive Man got mad because of that. After that I came to Omega to celebrate my leaving from Cerberus :D

I didn't think there was a timer either but for RP I went immediatly. So I couldn't finish some missions and I lost Jack & Légion. Even if they are some of my favourite character, it was really good to see them die, not knowing what choices killed them.

I always do all possible research and side quests before the next main plot mission, so I had a fully upgraded Normandy and no sidequests to distract me from entering the O4 relay immediately after the attack on the Normandy, saving the crew. Everyone was loyal, too. But I wanted Miranda with me on the squad, didn't trust Garrus to lead the fire team, and not knowing about Jacob I selected someone else to lead the fire team, and Tali died in the vents.

Kept the base intact and told TIM to use it against the Reapers. Was "rewarded" with that ridiculous line of Shepard's about not letting fear compromise who I am, and after that decided always to tell TIM to shut up.

I sent Thane through the vents because I figured it was risky as hell and he was dying anyway. Turns out closing doors is more difficult than climbing through heated vents. Other than that, everybody survived.

First time, my LI Tali got hit with a rocket because Samara led them....then I was so distraught over Tali that I had an "accidental" Mission Failure so I reloaded from the beginning and had Legion in the Vents with Miranda leading the fire team. Jack on biotics - Jacob on second fire team (can't remember who I sent with the crew - Zaeed?) then brought Samara and Miranda with me to fight the Reaper. Everyone lived.
