Got all the ship upgrades. No causalities.
Sent Legion through the vents. Miranda was the team leader, but was unloyal. During the Jack-Miranda conflict, I didn't have enough Paragon to resolve the argument, and I was like "Side with Jack=Paragon!" Thus, Legion died.
I waited to go through the Omega-4 relay, so about half of my crew died. Still not sure what I was doing... I guess I figured that in most games, even when the game tells you something is REALLY urgent, you can wait for however long you like to go and do it, and still have a happy ending. But alas, twas not the case.
I had Grunt lead the second fire team (though now I have no idea what I was thinking, he's an immature baby krogan). Thus, he died.
Miranda then died holding the line, cause she wasn't loyal. Then I blasted the base to smithereens, and proceeded to feel like a big goddamn hero.
Though in my 'canon' Shepard, I save the base.
What happened when you did the Suicide Mission for the first time?

my first suicide mission went not so well
. the only person to not be loyal was jack, because i gladly grabbed mirandas side of the argument after blowing up that research facility.
i must of missed one ship upgrade, because instantly jack was beamed to death while going thru omega relay lol. part of me was pissed because i thought my upgrades were solid, but i was wondering if jack not being loyal made her/him die, either way i didnt lose sleep over that thing dying.
picked thane for tech specialist, because i assumed he would kick ass crawling thru vents (he agreed that he has experience in this matter, and i remember he was in the vents while sneaking on Nassana). although i didnt know much about his character, and i dont even think he is a tech character lol. needless to say he couldnt close the door for in time, and his head was blown off :/
garrus led all my distraction/second teams, and jacob was the escort for kelly/chokwas. actually legion wasnt on this team (hadnt activated him), but the defense held the line good. only thane and jack died in the end, and both were expendable. jack had no friends and was anti-social "ehh my life sucks, kill me now", thane was awesome but his illness would have taken his life eventually anyway. atleast he died in honor

I had all the ships upgrades,but legion was not loyal as i didnt do his loyalty mission,miranda and jack fell out and as far as i can rembember i lost mirandas loyalty,so a few of my characters died
garrus kept dying after the hacking sequence,as did miranda right at the end,i have since started the game from scratch again,got all the dlc,got all loyalty missions correct (apart from zayeed) and everyone lived,horrah

I actually upgraded the Normandy without prior knowledge that it was beneficial. Something about the Thanix Cannon description talking about blowing up the Collector ship spurred me to get the shields and armor.
Jacob died from rocket to face. Now I didn't hate Jacob, he merely volunteered for the vent assignment and I figured "sure why not." Might have also had something to do with making Tali the fire team leader. Garrus and Miranda were in my squad and thought to myself that just because everyone around Tali (Freedom's Progress/Haestrom) died it wasn't any fault of her own.

I lost Mordin mysteriously on the way out. All squaddies loyal, but I don't think I had purchased Legion's upgrade because I got him right before I went to the Omega 4 relay. There was something on the suicide mission thread that explains why, I think. I was bummed. Have never lost anyone since.

Everyone on my squad lived and it was a good time.
I did all the upgrade because, "hey, may be it will be usefull later".
But i lost some crew member, kelly, and the woman enginier.... i didn't know it was related to how low you launche the suicide mission after the IFF.... i wish they made it better.
On my second playthrough i lost mordin, he was a not a tech expert .... (but he still make all the tech upgrade in normandy, he have his personnal upgrade as à +20% damage on his tech hability)... i though it was a bugs at first, then i checked on internet.... no tech expert .... face palm.
Only guy i lost so far i remember.

I lost nobody but Legion, the abduction of the crew made me chicken out of his loyalty mission.

I chose Legion for the door and he died because Samara was the second team leader I couldn't figure that out for a long time until they said if you pick poorly for the 2nd team whoever is at the door dies anyhow.
Samara died at the 2nd door even though she was qualified because of her history and background but BW didn't think so.

First time, I lost Mordin at the very end. Being a hardcore RPG nerd, I made sure to get anything and everything that could possible make me, my team and my ship more badass. Got every single possible upgrade. Went straightaway to get the crew (I just couldn't stand that emptiness in the ship).
Sent Legion in the vents, had Jacob be first team leader, then had Samara for biotic bubble, Miranda as second team leader, sent Grunt back with the crew (I thought it was logical...if I could send only one team mate to protect a bunch of unarmed people against a horde of aliens, I thought, what better choice than the perfect krogan brick wall of death?)
Took Garrus and Thane to meet the Reaper larva. Everyone was loyal.
In retrospect, if I would have even just left Garrus or Thane to hold the line, and/or sent Mordin or someone else back instead of Grunt, Mordin would have lived.
I was extremely depressed that Mordin died, so much so that I abandoned that character after the SM and will not play him in ME3 most likely.
Haven't had any fatalities since then.

My upgrades proved worthwhile as the armor held, the barriers held, and the cannon destroyed the Collector vessel.
Tali opened the door and Miranda successfully led the fireteam.
I lost the crew (except Chakwas, who was escorted back by Grunt).
Samara's barrier held and Garrus led a successful fireteam to the rendevous point.
Mordin and Zaeed helped me defeat the Human-Reaper while Tali, Jack, Miranda, Jacob, Thane, Samara, Garrus, and Legion held the line.
My entire team got out of the Base before I blew it to kingdom come.
"Unlocked: No One Left Behind: 75G"

Well, I decided that that weird thing called ''ship upgrades'' was completely unneccesary and only there to make me paranoid. Bam, Legion, Thane, Jack dead as hell. So when I got down there and I had to choose techies, I was like ''Well, Legion's dead, Kasumi doesn't exist and I'm not going to risk Tali, so Jacob, you voluntered! Have fun.'' and for squaddie ''Not going to risk Garrus, Miranda's a ****, Jacob's busy'' so Grunt got the honours. So bam, Jacob dead. And then Samara for Biotic and Miranda as squad leader. And once again, I thought that sending an escort with the crew was superfluos and only a way to make me lose a squaddie, so yea, half the team was dead too. I got through the rest of it.

I did all the upgrades, had everyone loyal aswell, However seeing how Zaeed was an experienced mercenary I figured he'd be a good teamleader... thought wrong...
Also, just about the whole crew died pretty much because of the same reason of silverignika... "i figured in most games, something urgent, is not really that urgent, so was still busy with the loyalty missions at that point.

Hit the relay as soon as Legion became loyal and then watched my male shep give Miranda a good time
Got my tactics a bit wrong against the oculus but managed to survive it's onslaught with the help of Garrus and Grunt. Then crash landed on the CB with no losses. Sent Tali through the vent and Garrus was the 2nd team leader. No one died there and we rescued all of the Normandy crew
Then used Jack for the bubble and took Miranda and Grunt with me and Garrus lead the other team. We all made it through to the heart of the base
Then gave the speech and took Miranda and Tali along to fight the reaper. Decided to destroy the base, Miranda resigned, kicked the collectors' butts, kicked the reaper's butt, we all survived and got back to the ship and o told TIM to get stuffed. Shep takes the ship and crew off for some well earned R&R
Me? I'm well happy to have finished the game in great style

I lost Tali in the last stand, even though I had her loyalty. So I ran it again and just took her with me to fight the human reaper. That time, everyone came back alive.

I figured there was going to be some kind of ship combat later on in the game when I read about the ship upgrades, so I got all of those, and i'm a bit of a completionist, so i went around and did everyone's loyalty mission before getting the reaper IFF. (except legion of course... did that right after)
on the actual suicide mission, i just followed directions:
sent tali in the tube cuz i need a tech expert, had garrus lead the fire team because I worried that choosing miranda might get jack killed. rescued kelly and the rest of the crew because i did the suicide mission right after getting the IFF. sent mordin back to the ship because he's a labrat, not a soldier. had jack be the biotic specialist and had miranda lead the second fire team. took miranda and tali to fight the reaper... miranda for the squad bonus and tali for the attack drone. blew up the collector base and miranda told the illusive man that she was resigning.
everyone survived. turns out i did it pretty much the way the wiki guide tells you to do it. lol

Everyone lived.. I destroyed the base out of "in the moment emotions". After I sat down and thought about the base.. and realized we need that base.. and with EDI, ME, and REAPER IFF projects being huge successes -- I figured I could trust TIM with it at least until the Reaper war is over.

I lost Kelly Chambers and half of the Normandy's crew because I did not head into the Omega 4 relay right after my crew got abducted ( I chose to go shopping on Illium and Tuchanka instead, I wanted to have as many eapon/armour upgrades as possible for the big mission ).
I also lost Jack in the SM, I did not have enough Paragon points to resolve the arguement between Miranda and Jack without offending someone and I chose to defend Miranda since I dislike Jack. everyone else survived. Destroyed the base and told TIM to go jump in a lake.

Everybody survived apart from the Kelly et al, I took to long before the mission since I was unaware of the PoNR at the Derelict Reaper. Chakwas made it though.

Since then, I typically make it a point to lose Miranda because I'd rather not deal with her in Mass 3 if I can avoid to. She's just an annoying ****.
