my understanding is that in-story you weren't meant to have the new scars either. The lazarus station was attacked before the project was completed, meaning that Miranda had to wake you up significantly ahead of schedule, before (I'm just guessing here) the final few procedures were done to fix you up cosmetically/insulate the implants properly from neural stresses, so either you to do the last bit of healing on you own slowly as a paragon, have your uninsulated implants burn open your scars and show through as a renegade, or have Dr Chakwas do the procedure with the upgraded medbay, which I assume is something like what Miranda was going to do anyway before you were properly woken up on Lazarus' original timetable.Legion of Grunt wrote...

...why would they remove scars he had for years but leave the ones that he received from the start of ME2 instead of removing all of them.

I think the final level of renegade
scarring went a bit too far - particularly since no one comments on
Shepard looking like they've been possessed - but I also wish paragons
could keep the initial scarring.
I know, right? I wanted some lasting blue or grey scarring for my Paragon shep, especially since he'd lost his epic most-of-side-of-face-bisecting scar from the first game
