...why would they remove scars he had for years but leave the ones that he received from the start of ME2 instead of removing all of them.

my understanding is that in-story you weren't meant to have the new scars either. The lazarus station was attacked before the project was completed, meaning that Miranda had to wake you up significantly ahead of schedule, before (I'm just guessing here) the final few procedures were done to fix you up cosmetically/insulate the implants properly from neural stresses, so either you to do the last bit of healing on you own slowly as a paragon, have your uninsulated implants burn open your scars and show through as a renegade, or have Dr Chakwas do the procedure with the upgraded medbay, which I assume is something like what Miranda was going to do anyway before you were properly woken up on Lazarus' original timetable.

I think the final level of renegade
scarring went a bit too far - particularly since no one comments on
Shepard looking like they've been possessed - but I also wish paragons
could keep the initial scarring.

I know, right? I wanted some lasting blue or grey scarring for my Paragon shep, especially since he'd lost his epic most-of-side-of-face-bisecting scar from the first game

Yes, they are there. I have my scars. And you can regrow scars for being renegade if you didnt use the med-bay upgrade.

If the char I imported used the med bay, are the scars lost forever?

No, you get them back during ME3 if you go renegade.

Dr. Chakwas tells Shepard that "stress" (renegade actions) causes the healing implants from ME2 to start rejecting. So yes, they return slowly, and to different degrees, depending on how many renegade choices you make.

I really wish the scarring wasnt tied to paragon/renegade. Why can't i be a good guy and have a cool scar? Or be a bad guy with just few scars. Its just like life; Looks can be deceiving. Otherwise all the murderers, robbers, thieves, rapist, child molesters would have red glowing eyes with a fat scar across their face.
Stupid scaring was explained away with "stress". Which doesnt make sense at all, since selfish and renegade actions are implemented by thoes w/o a conscience in the first place. Therefore there is no inner conflict within themselves to cause "stress".
For example; if you see a friend or family member who's acting a bit selfish, more likely then not, they are unaware of their selfishness.
I mean, you may choose to do something which is against your moral judgement because of the circumstances, but you arent gonna make 100 different choices against your own judgement. When we see someone who's regarded as "evil", they are not going against their own beliefs, but their moral scope isnt aligned with the general consensus of the society.
