I've been having this problem for the last few days.  I hadn't played the game for a couple months (version 1.02) and came back to version 1.03.  After playing for maybe 10 minutes, the game crashed while transitioning to a different area (during a cutscene in the Night Terrors quest).  I decided to leave the game off for the time being because I thought it might be the system overheating (our air conditioner broke down).

However, when I came back to the game after the system had cooled down, it still occured.  It doesn't happen every time, and it's at the worst just an inconvenience for now, but it DOES happen eventually, and I worry about what may happen to my computer if the game keeps crashing the way it does.

For info, I have the Steam version of the game, and my specs are

CPU: Intel i7 930
OS: Windows 7 64-bit
RAM: 6Gb DDR3
Video Card: nVidia GTS 450 (unless the drivers have updated within the last few weeks, I have the latest not-beta drivers)
Direct X 11

I used to have a sound card (Sound Blaster X-Fi Extreme Audio), but it was faulty and I ended up removing it (should've researched it first, been burned like that before).  The drivers for it, however, remain (I'm not sure how to remove them).  I've found a possible fix (for the crashing) via google (gamecrashfixed.com, Dragon Age 2 Problem #16), however I'm not sure if it actually pertains to my problem.

I usually run the game on High settings, with my resolution set at 1920 x 1080, with Direct X 11 as the renderer.  Anti-aliasing is off and I don't use V-sync.

To reiterate, the game ONLY crashes during area transition, specifically only after choosing a destination on the Kirkwall map (sometimes it crashes before the map loads, but not always), or moving to a new area from a dungeon, and ALWAYS before the loading screen appears.  Has anyone else had this problem?

Could you find out your specific driver version? There have been issues reported with driver 275.33 and DA2 on Very High graphic settings. However 275.33 only came out a week ago. ("system information" in the lower left of the Nvidia control panel will tell you)

edit: oh, I could have sworn you said you were running "Very High" settings. I blame that "V" directly underneath it. :lol:

Hm, well then. I'm not sure...

I'm eating my own words anyway. Turns out, a new driver version HAS come out in the last week. We'll see if that helps any.

Well, I wouldn't recommend it. It seemed to cause flickering shadows and crashing in my game, and some other peoples'. (see here)

Though on High settings it seems to work fine.

I only play on high, anyway. Last time I tried Very High, well, "hilarity" ensued. Anyway, installed the new driver and got rid of the bad sound drivers (downloaded and reinstalled the motherboard sound card drivers). This seems to have resolved the issue for now (rather stupid of me to NOT check for those until AFTER I posted).
