Okay, onto more ridiculous I wants realisms:
Enemies using the same powers as Shepard to varying degrees of effectiveness (on insanity, I want a careless Shep to get caught in a singularity)
Overload/Hacking having an effect on Shepard's implants(insanity only...not asking for Shepard to be hacked, just disoriented for a few moments)
Ability to buy stuff from the black market
Mild Aging, if the game takes place over a long period of time
Post Traumatice Stress Disorder FTW!
Getting sand/debris in our eyes (disorienting Shepard for 1.5 seconds)
Fatigue/Insomnia feature...Shepard must take a break after three or four missions, otherwise their alertness will suffer and aim may become slightly jittery. Because Interface screw is an awesome trope

Kinect/Certain Dialogue:Alerts enemies to your position
Since I realized that complete disabling of save feature might be harmful to a person's xbox or ps3 (being forced to leave it on to complete the achievement), perhaps for the sake of not harming our somewhat expensive technology, we should only have chapter saves, and less of them than there were in ME2.
