you think ? well they could make different scenes for those different situations in which are important choices etc and if you fail that mission , situation of the game etc you get that scene.

That would be better, still why would they go about making all kinds of different 'Oh dear, you just died' screens and images of what's going to happen now when the 'critical mission failure' screen works equally well. I wouldn't mind some extra visuals in addition to that screen but I'd prefer if BW spent that time on the actual game polishing.

I imagine it'll be a little more difficult to keep your favorite npcs and squadmembers from dying this time around.

Perhaps Shepard dies, and wakes up in a sort of "afterlife", with Kasumi, Miranda, Jack, Thane, Jacob, everyone from the Suicide mission crowded around him.
Shepard smiles, happy to finally be with his old frie-
POW! Jacob knocks his teeth out.
And Thane delivers a kick to the groin!

I think if you make enough questionable decisions then you should totally be able to lose the overall game. Admittedly, this might suck for people who aren't doing it on purpose, lol.

It's possible. Depends.
In the "fail" ending in ME2, for example, you still destroy the Collectors, it's just that Shepard and almost everyone else dies doing it. ME3 could follow the same model. Or not. Who knows?

(the only people ive heard that managed to have shepard die in the ending actually purposely did it that way)
