I'm fine with a non-standard game over, but it should happen as soon as I screw up. If I make a decision (or fail a mission objective) that results in the Reapers winning, then give me the game over right there. Don't let me play for several more hours before telling me that the game is now unwinnable with my current circumstances. At the very least, do what Morrowind did and give you a pop-up informing you that the game is likely unwinnable now, but you can keep playing if you want.
I'd just be pissed off if I failed hours after making a decision, because of that decision. It's a great way to waste people's time.
Can we fail?

But I want an ending where my Shepard can die.
I think it is obvious that if you kind of rush it, the chances of winning the war is slim like in the suicide mission. Still it is really hard to get Shepard killed unless you made the wrong - or right in this case - said choices.
But in my opinion? I think the galaxy will be saved no matter what somehow. I think it is the aftermath that varies whether it is favorable to mostly everyone or just humanity. The bit about "failure" comes from whether or not Shepard, the hero and driving force behind it all, is there with everyone else and reap the benefits.
Although I have to say I don't remember ALL the decisions I've made in both games. I just hope the repercussions of those choices don't hit me so hard for not remembering them.

Without a doubt a fail will be one of the emdiings judging by a complete team wipe in the last and the commander dying outcome.
