Garrus and Tali for sure, they've been with me from the beginning and they should be there at the end. Garrus is like a brother and Tali is my Shepard's soul mate. Wrex because he was one of my go-to squaddies in ME1. Thane because his sniper skills complement my vanguard very well and he's great for warp bombs. For the 5th, either Liara or the Virmire Survivor, because it just wouldn't seem right without one of them.

Really tough question, I'd rather have them all, but I'll try:

Jack, Thane, Samara, Kasumi and Mordin (three of the coolest, the cutest and the funniest). Hmmm.. I'd need a sixth for Wrex, though... Oh, and a seventh for Garrus.

Miranda, spent 2 years of her life bringing you back from the dead and is the hardest to kill in the suicide mission

Besides them, I'd love to get either Wrex or Grunt (maybe both?) back. Fingers crossed. Legion is also very cool.
