Okay. Here's what happened (possible very minor spoilers ahead)

I was in the deep roads fighting the golem boss and I died

I try to load my save from the death screen and as it is loading and says filed damaged, I look at the list of my saves and the file is gone completely of that character.

Anyone had a similar problem or know what it could be?

I saved a little bit before the fight but not during the autosave.

I had this happen in Danarius' Mansion, couldn't reproduce it (not sure how much of a good thing that is), but on the other hand after several hours more gameplay it hasn't happened since.

You said you got a damaged save file as well but since playing again you hadn't gotten it again, that was four days ago, how many hours have you been playing since, how far in the game are you and has it gotten damaged again?

Also did you die and then it said it was damaged when you tried to load it?

I am several hours farther in the game and it still has not happened again. To answer you second question, yes the damaged file was immediately applied/noticed on auto reload after party death.
