Well, Shepard is a cyborg now, right? Perhaps an explosion or something could blow off a large chunk of skin, while leaving the the metal skeleton intact.

In ME3, we'll probably be putting together Shep's criminal file for the Alliance, where we'll be able to reconstruct the Commander's face as we want. No need for a story explanation; you could completely screw with Shep's face in ME2, and everyone still seemed to recognize him/her.

This one would be the most likely reason. The Alliance apparently has weak systems.

You don't remake him just continue using the points and level we already achieved in ME2, which is the reason for the lower level cap in ME2.

Perhaps it is simply an intro for the trial where they present his file, the accusations and so on and you choose the appearance.

Shepard will be level 30 at the start of me3 if u use an imported save, and the level cap will bee 60.

how about he just does, no questions asked. If people want to complain about how that would make sense then they don't have to change their character

I really don't think an excuse is needed for character customization at the start. Less suspension of disbelief is involved than the whole Lazarus Project plot point.

They could just ask if you want to edit your shepard, Thats it.

Well, considering that in ME2, it was part of the plot, it's not really an excuse. The mirror in DA2 was obvious fan service, though.

Pure cosmetic reasons.  Shepard will get tired of waking up each and every morning to an old raggady man/woman in the mirror.  I hear plastic surgery is cheap in the future.

I will only accept this if I also get leather jacket, sunglasses, lever-action shotgun, and a space motorcycle to fly around on.

my shepard could use new hair anyway.... I personally don't mind if its just hairstyles that we can change.

His face mysteriously explodes while he's being held captive by the Alliance. The Alliance have no choice but to try and reconstruct it in time for the court case.
