*ahem* sorry, I'll just leave this here. This makes the assumption that the Crucible worked as expected, and that the Catalyst never happened. I also apologise for using a vanilla name, but I figure it's for the best. Just whiling away the hours, warming up the oven so I can get dinner going.
This is how my headcanon (obviously Liara-centric) ending sequence would go...
"You did good, child, you did good"
Jane opened her eyes and felt a dozen aches in a dozen places. Confusion replaced the pain, where was she? She tried to move her head but her neck protested, so she scanned her surroundings as best she could. The Normandy's med bay, or at least it looks like it. Was this a dream? Did she die on the Citadel, and now the final cruel joke of the afterlife is being played on her?
She heard the door open, and someone she thought she would never see again came in. Jane wanted to pinch herself, to wake up if this was just a bad dream, the Boy was bad enough, remembering Mordin, Legion and Thane, she didn't want Liara to be another nightmare, another ghost to haunt her. Liara walked over, and Jane felt her heart beat faster. If my heart's beating, I can't be dead, right? She asked herself this question as Liara came closer, and then she felt the touch of Liara's hand on hers.
Tears were running down Liara's eyes, as Liara put her head to Jane's hand, she knew this was real, she knew Liara's touch was real. "Hey" Jane winced as she said this, flares of pain shooting up her body. Liara didn't look up for a while, just holding Jane's hand. "I thought I'd lost you, again, Shepard" Liara said.
"Well, I did make a promise, and I knew how mad you'd be if I didn't come back" Jane said to more pain. But it felt good, pain meant she was alive. Alive was good. Jane's eyes feel wet, is this really it? Is it all over? Did they win? Anderson! Anderson's face flashed through Jane's mind, and she asks about him, before Liara could react.
"Anderson is... he's here. Jane.. But he's in a coma. Doctor Chakwas doesn't know if he'll recover, he lost a lot of blood, and there was a lot of damage from the... blast.." Liara wiped her eyes and let all this sink in, patiently looking at Jane's face, hoping for some sort of reaction."Damn it, I could have stopped him. I could have stopped the Illusive Man, Liara!" Tears fell down Jane's face, the pain of her final failure to save those she cared about hitting her. "Thane, Legion, Thessia! I could have-"
"No, no you couldn't. You did more than anyone could have, my love, you saved the galaxy, you saved us" Liara looked at Jane and stroked her face, wiping away the tears. "You came back" Liara leaned over to hug Jane, and kiss her forehead. "We still need that story for the little blue children, remember?" a small smile made its way past Liara's lips as she said this. Jane's mind raced, is she saying what I think she's saying? Are we, did we, have I really survived?
"Uh.. Told you we'd come up with something, Liara. How bad am I? It feels like I've been disassembled" Wincing in pain again, she managed a small squeeze on Liara's hand. "Karin said it's a small miracle you survived that. The blast nearly fused your armour to your skin, you've been out for three days. Most of the crew has been by.. those who made it. Your mother came by yesterday, she would love to hear that you're awake now. Joker wouldn't leave your side, he felt..
responsible, somehow. EDI took him back to his bunk. Earth is still cleaning up the Reaper bodies, and it may be a very long time before it will all be made right, Jane" Liara's eyes betrayed the pain she felt at this report.
"What about the other worlds? Thessia? Palaven? Did we save them?" Liara looked at Jane with a mixture of pride and pain. Even in that state, her spirit was unbowed, she was still concerned about her mission. "Initial reports say all the Reapers have become... silent. There's no activity from them, not since you used the Crucible. We can focus on rebuilding now, Shepard, the worst is over"
