If the intention is to get as many people killed as possible but for Shepard to survive, I can't imagine anyone getting that result by accident. Aside form skipping every loyalty mission, it does require a fair amount of arranged deaths to pull off. Not necessarily picking the wrong people for the job, but doing so alongside taking those characters who are able to survive regardless of their loyalty and getting them carried away by robotic bees.
Which I imagine was entirely the point, if everyone/the majority are getting fairly significant face time in Mass Effect 3.

So yeah...you don't really have to be deliberate. You just have to be rushing. I could see someone being likely to do the DR right after the CS mission and then end up triggering the crew being captured and rush after them with alot of loyalty mission and recruitment not being done. It's not that big a stretch.

Didn't the meta data say some significant percentage of players never bothered to even finish? (or was that da:o?)
If that's the case I wouldn't be at all surprised that people just blew through the game and didn't bother to do any sidework to ensure the party survives.
BUT that might also be why Bioware decided to include s/s romances this time. Gaider explained quite well why Bioware might not include s/s romances when that was still a fear for DA2. The minority option just isn't cost effective and the money instead gets used to provide the majority with more options.
But then if the numbers come back and it turns out the majority didn't even bother playing any romances then that stops being cost effective. If you know your entire romance audience is already a minority of players then including the minority option becomes more logical.
All speculation of course. Just trying to stay on topic

I'd fully expect The Masses to lose a few people along the way. I certainly can't blame them for not bothering with upgrading their ships fully. Mining, urgh. Losing as many people as possible without Shepard dying as well (meaning one survivor, and allowing the save to be imported) takes a bit more calculated work, however.
Still, I suppose. Can you actually set off on the suicide mission without recruiting everyone, out of interest? I've never tried.

True and since the masses will be the majority of players they need new people.

My bad, I thought you were talking about Vega vs Ash.
Yeah, she probably will be more specialized in ME3 -- at least I hope so. My wish is that every character gets at least 2 abilities that are distinctly their own. Kind of like after completing loyalty missions in ME2. This will really make a change as to who you would bring on a mission

You can go in without recruiting two out of Thane, Samara and Tali since you only need one of them to be able to trigger the suicide mission.
But since everyone is apparently getting a "previously on Mass Effect comic" with ME3 those statistics don't matter all that much and the only reason they really need new characters is either plot or marketing neccessity and let's just say I firmly believe Vega falls into the latter category.

Well If you're going to insist on importing a savegame with the minimum number of people alive and all the worst choices made I daresay the consequences should be yours, view it as a challenge that adds replay value if you must. In fact I plan on importing my totally Renegade FemShep who has done exactly that and I do hope she has a far more difficult task of saving the Galaxy (if it's even still possible) than those characters who made more reasonable and less rushed and hasty decisions.

Whatever you do in the previous games or the comic you can never have less than the minimum required two squad mates even without any new characters since You'll always get Liara and either Ashley or Kaidan.
would that leave you much choice, no would it make the game at least slightly harder, probably but it would still ultimately be playable.
