I would LOVE for the blue/red options to actually fail once in a while. You'd still get the paragon/renegade points for trying, but there are some situations where sweet talking shouldn't be enough. However, I am sure a great many fans would be ticked it that were to happen.

This is a worthy idea, but I don't think Bioware are in the habit of making their choices too obscure or having unforseen consequences. Mass Effect and DA2 practically telegraphed that you had to do certain things to keep squadmembers around; it's even easier in the Mass Effect series due to just one paragon/renegade meter instead of 6 or so different individual friendship/rivalry bars. 

I get why people want tough choices, but at the same time putting two squadmates up against each other for no discernible reason, and when the entire galaxy is threatened by the Reapers, seems a bit contrived. Surely they'd band together even temporarily, to help against a common foe?

As far as it relates to S/S (since that is, y'know, the topic...) I doubt either Vega or the new female squadmate will leave Shepard abruptly, we have little time enough with them as it is for Bioware to develop their characters. Ditto with the VS, I think a great many fans would be annoyed were that to happen.

I never romanced her and she actually came back. I didn't even know she could run away and never return until my friend said he "missed her" for the last parts of the game. I guess I befriended her enough where she came back with the book to keep me safe. I don't know how many people know that can happen though...

Except when it makes sense.

During WWII, while the USSR was on the same side as the US and GB against the Axis, at no time were the various armies under the illusion that we were friends. The USSR was suspicious that GB and the US actually allowed thousands of Soviet troops be chewed up on the Eastern Front, and I believe that was indeed the case, and a wise coarse of action as well given that the USSR was originally part of the Axis and Stalin was every bit as evil as the German leader Bioware won't let me name.

I can see the Geth and the Quarians suspicious of how many casualties each side will take and how it could leave them vulnerable to the other after the Reaper invasion. I can see the Alliance and non-indoctrinated Cerberus people jockying for position on how to lead humanity after the war. The Rachni and the Krogan have a history of hositlity, and both sides suffered at the hands of the Citadel races.

I can see Shepard convincing everyone to fight on the same side, but that some factions will be "loyal" and the others just along out of necessity. As was the case with "loyal" and unfocused squad members in ME2, the effect could be disasterous even though the Shepard can still win the game.

It all depends on how well it's written, and such rivalries could really add emotional weight to any Tali, Miranda, Ashley or Legion romances (stay on topic, stay on topic). But I WILL be pissed if all Shepard has to do is give a magic speech and everyone suddenly realizes the power of love and starts singing Kumbaya.

No, that's a bug that has been fixed. If you had a fling with Isabela, the epilogue of BOTH Isabela and the "real" LI would play at the same time with Isabela's playing the loudest.
