Miranda being second-in-command of Cerberus doesn't make a lot of sense considering how in the dark she was regarding some of the more nefarious things Cerberus got up to. If she was in second-in-command she should have been both more in the know, and should have been more directly involved in some of them. It makes more sense I think for Miranda to be an important asset for Cerberus, and a high level manager, but not part of TIM's inner sanctum. She should be high level but not top tier, and only knowing what TIM wanted her to know.
Making Miranda second-in-command by default makes her much more culpable in Cerberus' crimes. Whereas before (as of ME2) I think she was more of a more or less good person who allowed her ideals and 'results-oriented' nature to get her caught up in organization that wasn't quite as noble as she viewed it.
She wasn't all that "in the dark", considering how she knew of quite a bit of Cerberus' operations (Despite security protocol dictating that the various cells be kept separate from one another). For example, she knew of Teltin, the rachni breeding, the husks, the thorian creepers, the Grayson incident on the flotilla, etc.
