IMO, Miranda wouldn't want children so long as she's in her current line of work.
With that in mind... I'm not sure. Certainly by the time ME3 is over (Not counting the retarded endings), Shepard is a celebrity, and loaded with money. He's certainly able to support a family, at least financially. It depends on why Shepard isn't ready.

This, this and this. I said it back in the ME2 thread already I think, though there different concepts for the female Ghost armor, but Nova's is probably most known.
I forgot about Nova. Nova is a good pick too.
I really like the armor, I use it in Fallout 3/New vegas.
I just tell myself the Enclave made it in F3 and that I found it in the Old World Blues DLC for FNV.

The middle one is male obviously, I am not sure if the left one is Ghost or a Specter. Still would work imo, bit like the nanosuits in Crysis. I looove dem scifi suits.

The one on the left just resembles a Nemesis from the ME3 MP, just with a low cut top and exposed skin on the arms. The right picture more resembles the Crysis nanosuit, what with it's muscle-weave textures and light grey finish.

So he can die 1 of 3 ways. A slow, very painful death from the mustard gas, or a very painful death from an exploding bladder. If neither of those do the trick, then he'll die from the water oversaturating (effectively "drowning") his organs.

Medi-gel wouldn't help anyone survive a skinning, much like how it wouldn't help someone who's been decapitated or bisected.

If she did I would say I support you on having a child, but not right now there is too much at stake for us to think about children yet, but once all of this is over then we can seriously talk about children.
But too be honest during ME3 she might mention it at best but she will defintely say not right now but once everything is over (unless she is worried than you won't surive and once one night with you before you go to fight)

Another good point (I should have included in my post earlier). With the whole "interplanetary apocalypse" thing going on, it's probably not the best time to conceive. She may mention the possibility once or twice, but no action would be taken during the game. If she wanted children after everything is settled down though, then by all means, Shepard and Miranda can have children.
@bold- If there was a sex scene with Miranda in ME3, this is what I would place it in, in terms of context.

My Shepard would do to Kai Leng what Michonne did to The Governor in "The Walking Dead #33". Just worse.
