You have four "squishies" that must be protected: Kasumi, Mordin, Jack and Tali (everyone else can protect themselves if loyal). Every one of the three "heavy hitters" - Grunt, Zaeed, Garrus - can protect two of them if everyone is loyal. That means with everyone alive at the start of the final fight, without Zaeed and Grunt on the squad, two are left unprotected and Mordin will die. If you don't have Kasumi either, there's still one point less than necessary and Mordin will die.TheMarshal wrote...

It's not really a decision, there are just some characters who are dead by default in non-imported games. Wrex, Grunt, Jack, Thane and Legion, to name a few.
Miranda is alive in non-imports, but seemingly cannot survive. (Except some people have said that she has and does, just not enough for me to be confident it is true. Of course, most people around here play imported games.)
Anyway, if we are to include "imported game" as a survival condition for Miranda, it seems fair to note that many other characters are not even alive in that scenario.
I think the problem is there may be conflicting viewpoints here:
(1) Bioware may see this setup as an incentive to play the other games, for in spite of the marketing talk of ME3 being the best place to start, I cannot imagine they'd think playing ME3 without the other games makes for a well-rounded experience of Shepard's story. From that viewpoint, I support the decision to make the setup for the default game non-optimal.
(2) On the other hand, for a fan looking at the default setup, having characters dead seems like a statement that they're inconsequential for the full experience of ME3, especially considering the marketing campaign was targeted at new players. Which may be true in some cases but that's even more insulting, because if it is that way it is because they were written as being inconsequential. From that viewpoint, having ME2/ME1 characters dead in non-imported games seems like adding insult to injury.

I don't know if there is a mission limit. But I do know that if you start Thessia with no side missions not yet done, and do the single side mission which becomes available after Thessia before going to Sanctuary (N7: Communications Hub), you won't have problems with Miranda's survival. It's a long habit of mine to always do all side missions before the next main plot mission, and that worked perfectly in ME3. I don't think scan-and-fetch tasks count.ENorman94 wrote...
I saw this posted somewhere, but do you think that her survival may also depend on how long you wait to go to sanctuary? Ive always gone right away, but maybe if you do 1 or 2 missions before, she might die despite doing everything to save her. Just a thought. Doubt it. But might try it.

That "dont skip any of the meetings" part isn't true. In my first playthru, I missed the very first meeting at the Normandy docking bay. That's also the meeting where you get the codex entry and if u dont take that one, you don't get the entry at all. She still lived.
*Miranda must be loyal from ME2 (evidence to the contrary to be confirmed)
*If romanced, you must not break up with her (unromanced is ok)
*You must read Anderson's e-mailed dossier about Kai Leng before speaking with Miranda after the Citadel coup.
*You must not skip the vidcom conversation with her in the Spectre office after the Citadel coup.
*You must meet her again before starting Sanctuary and give her access to Alliance resources.
*You must have enough reputation to persuade Henry Lawson at Sanctuary OR take the Renegade interrupt.
This looks like a lot....

In january 2010 when I've seen the Loyalist trailer video for the first time
In the game, my Shapard began to feel emotionally attached to Miranda after the second conversation. And he fell in love during her loyalty mission.
