According to Weekes (if that really was Weekes in that semi-anonymous forum post, which it probably was), he had to successfully argue in favor of the squad member farewell scenes for them to even happen.
That really shows how far these guys had their heads up their *sses. I don't really mean that in an insulting way, I almost feel bad for them, but they really got it all wrong.

Now, if we had gotten a Hammerhead or a Mako to drive through the Conduit like we did on Ilos, that would have been neat.
Granted, I really wish Earth had got the same treatment as Rannoch or Tuchanka where there were a few smaller missions that needed to be done before the final push. Maybe have the Normandy and a pair of Normandy-class stealth frigates from the Fifth Fleet drop Shepard and a few Special Ops teams on Earth to lay the groundwork for the allied fleet's offensive then Shepard can call in the cavalry.

Earth escape takes place in Vancouver. Never heard anything about New York.

That was more of a "what you'd see via the news reports" type of thing. The same ones shown at the very beginning of the game when the Reapers first arrive.
There's also a mention of New York by Vega on Menae

Horizon is a priority mission (felt more like a side mission but we've discussed this), those don't have timers like the side missions do.

I think that DA:O developer said it well, not long after the game was released and the endings fiasco began. Basically, what he said boiled down to: they forgot the basic principles of how to make a roleplaying game, or deliberately ignored them.
I think that's a big part of the problem. With the two main areas being:
1) Your choices matter and affect the world in a meaningful way.
2) The relationships you build with characters along the way also matter. And the player gets to decide which ones are most important (within certain parameters, inevitably, of course).
I think they basically threw out the whole idea of player agency, more or less consciously, and it became all about shocking the gaming world with their uncompromising vision, or some such nonsense. So much so that even the rest of the development team was kept out of the loop.

Except there has been 4 different Miranda threads, totalling almost 4000 pages in all. Tali only has the one, IIRC, and the only reason hers grew so rapidly is because hers (and Liara's) are the two that receive regular, very rapid posts.
