It's placed on the wall, yes, but that's because they don't know Shepard survived. It is implied s/he did, or at least, you are given the option, as the player, to envision that.

But to everyone asking, what I think it symbolizes is that Shepard is not really dead, and you have a choice. Pretty clever by BW there, I must admit. The rest is up to us to headcanon regarding the breath.
For me, Shepard lives and reuinites with Miranda at long last. Miranda cures her infertility, and they get married and have children.
Yes I know I like happy endings.

Yeah, but with the way the Extended Ending is Constructed, it's difficult to Imagine on my part. Ther should've shown the Breathing Scene in the Beginning.

Overall, I'm happy with the endings. They give enough of an explanation of what happend while leaving enough ambiguity for us to fill in the blanks.

That's why her hands are clasped together.
