A brutal and heartbreaking decision to be sure, and one on which people will widely disagree. For which reason I'll be shocked if it doesn't show up in ME3 
For additional tragedy, the more people you sacrifice to save your friend or LI (whether Miranda or someone else), the greater the likelihood that she herself will be too angry/guilt-ridden to continue the relationship with you. Though, at some point - especially if she doesn't *want* to be saved at such an expense, and you do it anyway - I suppose this simply starts crossing over into selfishness.
What's this obsession with guilt? I have never understood it. I'd be glad to be alive, that's for sure. Also I make no excuses for not valuing all lives equally. Sadly, it is impossible to selectively kill the idiots. People like Delan from Horizon, for instance.
I think Shepard deserves one indulgence for saving the rest of the galaxy. Again, I make no excuses for it. I do not expect, however, that we will be put in a position where we cannot win.

No, that would not be interesting, that would force an unhappy end - death or break-up. About the last thing I want in ME3. Also I need Miranda to deal with Cerberus. She is not expendable.
