Now, do you really have to romance Miranda to have her live? Because I still warn her of Kai Leng, and give her Alliance resources. Yet she dies every time. It's like not being with Shepard removes that last bit of will for her to live.
In a default ME3 playthrough, Miranda will always die
She will live if her loyalty mission is completed in ME2. She does not have to be romanced to live.
tell her about Kai Leng
give her access to Alliance intel
when facing her father on Horizon, choose the paragon or renegade dialogue on the left
I know I completed her loyalty mission in ME2 and romanced her, and I fulfilled the other two requirements. Only I did differently was choose the renegade option on the left. She throws her dad out the window, and then dies.
I guess you talked with her on dock 24 when she mentions her sister? I also guess you didn't end the romance? If you did end the romance, she will die.

Why end the romance? Why not just play the game with a file that doesn't have a pre-existing romance with Miranda? I only say this because, speaking in-universe, Shepard has no reason to break it off with Miranda.
So I'm picking up where I left off in ME3, just to finished Sanctuary. Miranda is dead...though I always liked her death. She killed her father, saved her sister, tried to stop people from going to Sanctuary, she died a hero. I go back and forth between her and Ashely from time to time. But her death is a good one. Really redeems herself from being that Cerberus ice queen.

You have to play Shepard as a real a**hole to ditch Miranda anyway. She actually cries when you break up with her (the only romanceable character who does, I think), but not in front of Shepard (of course).

To be fair, you can always find a legitimate excuse that is in-universe to explain this.
For example, Shepard might decide that he wishes to put all of his focus on the war and prefers to distance himself emotionally from Miranda.
Or someone's Shepard believed he had a fling with Miranda and falls in love with someone else.
Obviously anyone that does that is not wrong (but definitely less right), it's just how they decide to role play Shepard. The power of headcanon is a great one.

Well the Renegade path kind of does no ? Or at least it doesn't contradict the possibility. (It honestly has been a long time since I've seen it)
and well....you can...headcanon that Shepard has Schizophrenia. ME3 limitation of dual choice and amount of auto-dialogue kind of supports that actually.

Maybe at the very start, but by the end of the game it's pretty clear that Miranda and Shepard love each other.
And yeah, I'd rather not be reminded of ME3's horrendous handling of autodialogue.

I think I have identified the problem here.

Which game?
It is quite possible to play her ME2 romance arc without it ever being serious. It depends on which dialogue you choose for Shepard, but the game does provide the option of Shepard defining it as a fling. The characters still care about each other of course, but it is possible to have a casual sexual relationship with someone and still care about them. I'd even argue its necessary, at least if that arrangement is healthy.
ME3 is a different story, and seems to roll with the assumption that the two were more seriously involved.
