Just noticed an interesting tweet by Mac Walters:

To me, this implies that the Shadow Broker (now Liara) is going to have video proof in her little video room that you cheated on someone. What that means for Ash/Kaidan I don't know.. But If you romanced Liara in ME1 and cheated on her with Miranda in ME2, she's in for a hell of a show in the engine room. Maybe we'll even get to watch it and see some goodies that they cut out of ME2 ;-)

Liara already knows if you cheated on her in ME2 though. No need for her to bring it up in ME3.

I think it would be technically possible for her to still die, if you left her to hold the line with a totally decimated squad. But it would be very difficult, to the point of being essentially negligible as far as the number of games where it occurred.

If you restarted Liara's romance in LoTSB and you just so happen to be romancing Miranda, then you're cheating, there's no room for interpretation. 

On the other hand if you didn't restart Liara's romance in LoTSB then Liara wishes you
well and she and Shepard remain friends. Judging from how the Asari view
relationships (long-term to them isn't really an option for the most
part) there isn't going to be much of an issue.
