Well, that was obviously a conscious choice. Coupled with early dev comments about playing the ME1 and ME2 romance interests off one another in ME3, I think that is another strong suggestion that Ashley/Miranda was the plan, at least at one stage of development (these things can change).
With Ashley/Miranda you really get a microcosm of ME1 versus ME2, not just, or even mainly, from a romantic point of view: it's the whole Alliance/Cerberus tension as well.
What that would mean for Miranda's appearance... I'm not sure. Maybe some sort of redesign. Or maybe certain visual similarities to better underline the rivalry/philosophical differences.
