I'm going to guess you get that line if you didn't romance him in ME1 and possibly if you "cheated" in ME2. Cheating seems to revert a lot of the romance back to square one dialogue-wise.

I had to fanfic it because it bugged me so much. Even ASH gets all worried over Shep - she brings up Liara, but she has a nice long 'it's okay Skipper' moment too.

No, he actually says that line only if you romanced him in ME1, because you can't talk about Horizon if you weren't in a romanic relationship with him before.
The line from the picture happens right before he says "I just want you to know that I'm not seeing anyone".
After you speak to him the first time he's awake, after he says the line about "there's a war on", go back and talk to him again and choose "Horizon". That's when the other line comes up.LadyofRivendell wrote...

It's totally OOC for Kaidan not to show any concern for Shepard after Thessia. The only LI's who matter are Liara because bioware loves her, and Garrus because supposedly everyone loves him
