Actually, there IS a fanfic that is "what if they knew each other from basics?" kind of thing (it does follow the game universe to a certain point)

All the Things That I've Done is a very good one (and still in progress) that has them meeting several times before the events of ME1.

This was my impression as well. Shepard, as learned the other day, is one of only a few successful graduates of the N7 program, and had been involved in some pretty heavy **** during his/her short tenure with the military. It would be difficult to not at least know of Shepard.

I agree with you.  Considering that Kaidan entered the Alliance a year after Shepard - Shepard entered in 2172, Kaidan in 2173 according to the timeline in the Wiki - they couldn't have done basic together.  Though with Shepard being N7 spec ops, it would not be entirely improbable for them to have crossed paths from time to time, though I doubt it since the Alliance Navy is huge.  I don't know, while I can see them meeting prior, I just don't think it happened.

I think Kaidan knew of Shepard as did everyone else from whatever background heroics you chose, and I would bet Shepard as the Normandy's XO would have read all the personel files, especially those of the Marine contingent since that would probably be the group she worked closest with. 

Also there is the probability that even though this is the Normandy's shakedown run, if they follow the same protocols as today, the crew has been involved with the Normandy for a good part of its final construction, so most likely Kaidan and Shepard could have been working together for close to a year.

NOTE: This is my thoughts on this and only my opinion based on what I know with the US Navy, which it appears that at least some of which the Systems Alliance Navy is based on.

As much as I hate to say it, I'd say the status of Kaidan/Shep's "relationship" pre-ME1 is a case of "Speculation for everyone". I can make a case for them simply knowing each other by reputation, and I can make a case for them having had a (slightly) longer acquaintance pre-Eden Prime. Certainly, the way they fall into a very easy camaraderie tells me that there very well could be some history there - even if it's just a few weeks spent prowling around the ship while she was being readied for her shake-down.

They can't have met before, or my head canon and the end of my pre-ME1 fic in which Anderson introduces them on the bridge of the Normandy would be wrong.

Bur I, too, think Shep had been famous within the Alliance and this was how Kaidan knew of her. I also suspect that he had a sneaking sympathy for her, as someone marked out as a bit of a freak for her actions if you choose sole survivor or ruthless, maybe even war hero as well.

I could see that. Yeah maybe not missions but been around each other for a bit on the Normandy. And I'm sure Shepard has read up on her crew. While walking around the ship everyone seems pretty comfortable with each other. I guess we really don't know how much time has passed between Shepard looking out the window scene and the start of the game.

She also let him go too...easily, imo.

Yes, he does. She does remark though, instead of "you're in the presence of a legend", she says "You're in the presence of a God". It goes pretty much the same. He had the same dialogue options.

Yeah, mine too.  I refuse to believe.

But she's still known Kaidan longer than any of the other squadmates, even if only by a few weeks.  And Joker, but he's not really a squaddie.  The way that Jenkins talked about her in the prologue leads me to believe that most of the crew knew more about Shepard than she did about them by virtue of her reputation.  I don't know if Ashley recognized her when they picked her up, she never really let on.
