It's my understanding that the import bug is part of the aftermath of EA's withdrawal from the use of Steam as a platform for game-delivery and the creation of their own Origin program.
Basically, if you bought and played the first two games when they were available on Steam and then tried to import that game into ME3 (only available on Origin) you received an error saying you could not import your original customized face. At that point you had to try to recreate your Shepard from scratch using a completely new graphics pack with varying results (I still haven't got it quite right). That's at least what I've been able to discern, I play on a pc, it could be different for x-boxers.
It's not just steam. I bought my two games out of the pc box, and I couldn't import my ME1 Sheps. So I originally only had 1 character I could play out of my four Sheps. In ME2, I thought I'd give one of my Sheps a different hairstyle and didn't realize that it would totally reset her look, but in the end, she wound up with an ME2 code which made her work with ME3.
That was poor Zaira, my ugly Shep, and my adept. I just explained it away as Lazarus reconstructive surgery.

However, none of the other three would work with ME3. There was a post running around the forums where someone took the time to figure out how to use Gibbed to reconstruct a pretty close approximation of what your ME1 face code would have been in ME2. So, I spent all of the two days prior to ME3's release with Gibbed, a calculator, ME2 and the ME3 demo reconstructing me ME1 faces.
In the end, I would up with two of my four who went with BioWare's fix when they finally fixed the import problem - those would be Kelly and Aili (Aili actually came out pretty good looking, so she did get changed while Kelly stayed pretty close on the whole). I kept Jade with my own reconstruction liking it better than the fixes suggestion. And, Zaira just needs help period, so nothing made her better.
