Where BioWare fails is not letting Shepardreally explain his-/herself, she just stood there and let the VS rip into her/him. At the very least there should have been an option to say

My bad, always thought Chakwas survived. As I said above, it could have been made so much more exicting.
When I replayed through it, obviously Kaidan is angry but it didnt seem as aggressive as I thought, or maybe I'm just numbing it out...

You'll definitely want to grab most of the minerals and artifacts on planets you are on for side missions, and it definitely wouldn't hurt to kill most bad guys on foot rather than the Mako, and the DLCs will certainly help, but my fairly extensive testing as determined that as long as you are 54 to start the NewGame+, you can definitely get 60 (it may be that even lower than 54 will do it, I just haven't been willing to risk it).
I landed lvl 60 on my second play through coming off lvl 50. It is tedious work. Killing everything but thresher maws on foot. 59-60 feels like it is never going to happen. Just stick with it and you'll get it.

I had that same idea, but someone once mentioned to me that Shepard would be more likely to go to the Collector Base as soon as Kaidan got captured. Hell, I know I would. "Screw upgrades and loyalty missions! I'm going for you, Kaidan!".

Hmm, I had Liria and Kaidan with the rachni queen, and kaidan there wanted to kill her yes (as renegade) - but with normal kaidan haven't had him do that lol
Same with counsel, normal save them, renegade he wants to let them die. Hmm, will have to try both again just to see if it changes lol

I did 'paragon' him though.
I think his line was like "This is more than about humanity" not 100% but...
