I'm guessing it means Miranda took Ashley to Project Lazarus in secret. Apparently she thought Ashley could use a few cup-sizes extra and here she is carrying her to some alley so she won't know what happened to her, looking quite pleased with the result. Dr. Robotnik was obviously helping out. Obviously.
He'll probably be the main antagonist in ME4. He was a major backer of Cerberus.

Ehh... Not sure if serious. If I had concussions, what should they do, just stand up and leave? Amnesia is still present - I barely remember my childhood and youth, my friends or family members reminding me of some events, and I do not remember them (events, not friends or family members). And nightmares... Well, most of the time, they are present only when I sleep, but that's not happens often, because I have insomnia. Unless somebody uses paint or laquer in poorly ventilated areas, of course.
