Both Shep and Ash are adults who know that in the grand scheme of things the status of their relationship is kind of an after thought. I don't think either of them were thinking much beyond being happy/sad/confused/angry/speechless when they saw each other.
They're clearly not "together." Haven't been since Shep died. I'm not sure they were ever really officially anything like "girlfriend and boyfriend." I don't feel like they got the chance to really get to that place before everything went to hell. They cared for each other and liked spending time together and they each probably suspected that this was "love," but like I said it didn't really get a chance to grow into anything beyond that.
That's what makes things so weird for them now I think. Think of the questions Shepard has: What has she been doing in her life? Has she moved on? Is she over me? Was what we had really so special? Ash probably has some of those same questions.
I think its far more interesting to have left their newly blossoming relationship in the somewhat new and discovery phase rather than to have them clearly been in a mutually deeply committed relationship. With the latter, there is no question when you see that person again that you still love each other. With the former, there is a bit of trepidation, questioning and possible re-exploration. That's what I'm hoping for.
In any case, John is stupid for Ashley. But in his own way. He's put the whole thing to the back of his mind, hoping that one day soon they'll actually get a chance to clear the air. I dare say in ME2 he even may have had a slight connection with Miranda, which he would probably never admit to himself or even notice really because he's' got one woman on the brain.
I can't even fathom how Ashley is feeling. I'm sure there is a part of her that wants nothing more than to just hug him, kiss him and pretend the last two years never happened. But Shepard is also the source of a lot of pain for her (who knows how much) in the last couple of years. There may be a part of her that was finally, FINALLY okay with letting him go and then here he is like a punch in the gut. That part of her might think its easier to simply walk away from it all.
Yes and no it may be a bit tired but if we make that kind of thing secondary don’t we lose, or risk losing, our reason to fight, this can be said of life on an individual level.

From what we where shown no they may not have been. Though at the same time if you take that and assume that they had feelings to match their actions, to me, they become BF/GF maybe they never got to the superfluous knowledge that helps us build a full relationship but the deeper connection seems implied, imo, if only because they broke such strict military code.

She could be anywhere I hope it’s still in love with Shep though. Provided we are given a chance to convey our intentions one way or the other, without her being hurt.
