I might orchestrate Kaidan's death on purpose in ME3. I like tragedies. Maybe even let Garrus shoot him.
My second FemShep was in a similar situation. Romanced Kaidan in ME, turned to Garrus in ME2 and chose at least some renegade answers during the Mars / hospital dialogues, including the "our fight at Horizon really threw me" (or similar) line, which I suspect to lock out / end the romance permanently. I made a pre-elevator Citadel save, to test the different options in the Udina-confrontation later, with the intend to let him die - Shepard was really angry at him, after all.
Turned out I could not do it. Not because of Kaidan, but Garrus seemed to be more than upset when he had to witness or, worse, cause his death. Well, a reload and the right conversation choice afterwards turned the Major into a war asset and saved the Turian from emotional trouble - Shepard could live with that.
