Its quite normal that certain positions in military come together with a promotion in rank as the one holding that position should have at least rank X.
Its also quite normal that you skip some ranks between your current rank and the rank assigned to the position.
I find it more surprising that she only became Lt.

I think in ME2 TIM tells you that Ashley/Kaiden got promoted. So no news here.

They got promoted in the two year timegap between ME1 and ME2, yes. But at least Ash got promoted again between ME2 and ME3 if I get that right.

Ash was Gunnery Chief, was promoted to Operations Chief and is now 2nd or 1st Lieutenant.

Kaidan was Staff Lieutenant, was promoted to Staff Commander and with that he is actually Shepards superior.

Kaidan/Ashley are made Spectres after 2185, either just before or after the Council is aware Shepard's alive. Remember Shepard defeated Saren and died in 2183, and the Lazarus Project took two years to rebuild him.

We don't know when exactly they were promoted, but we can guess.

Promoted once sometime after the Normandy blew up. Could have been a spectre that whole time as well. The spectre stuff is fuzzy. The higher military rank at the very least comes sometime after Horizon.

Here also.

First one says that "by the time of the events of ME3 Ashley has become a Spectre." The Spectre article also says it was after 2185. That means Ash was given Spectre status only recently. I wonder why.

Shepard defenately deserves a promotion. However, he is probably going to command a fleet of ships later on. We already see in the promotional video at E3 that he has the quarian fleet at his disposal.

Why not make him a Captain at the beginning of the game, perhaps give him a battlefield promotion (by anderson when the Normandy arrives). Then when the game progresses and Shepard is to command a (small) fleet, they promote him to Commodore. Depending on which military you take as a refference, commodore is a higher rank than Captain, but usually lower or equal to Rear Admiral (depending on your source (see on the ranking colomn on the right)).
Rank: Commodore
Other names for Commodore is 'Fleet Captain', or 'Senior Captian'.

I dont think that a Commander (of whatever grade) is going to get command of multiple ships/fleets.

Next to that, Commodore has about the same ring as Commander, only more bad-ass. :)
