Wreav seems to only be a replacement and a plot device if Wrex were to die. Not much is ever said about him, nor does Wrex mention him if he lives. Could he have possibly been killed off or what?

I guess he was the leader of Urdnot, until Wrex got home and took over.

Who knows what happens to him if Wrex is still alive. He should have still appeared even if Wrex was alive(his second in command perhaps?).

Wrex has progressive ideas for the Urdnot, Wreav didn't.
In a civilized culture, you'd sit down and talk about the difference in opinion.
In a krogan culture, what the heck do you think happened?
Wrex left and Wreave was probably in control of the Urdnots. If Wrex is still alive and came back, and wanted to take control of the Urdnots, I doubt Wreav dusted off the throne and offered it up freely.
My guess is that he's a set of bleached bones somewhere on Tuchanka now.

There's no indicator Wreav was ever in charge of Urdnot pre-Wrex's death, in fact he says he says he took over AFTER by becoming friends with his brother's friends.
