Is it a bug? because on my latest playthrough I made sure that everyone has my loyalty and I even made sure I had enough Paragon points to cover the little moments that pop up that might ruin reputations with crew and I had my ship fully upgraded. Still Mordin died.
I just cannot seem to make it through with everyone alive!

But, if you don't have the time, for now, and you have a prior save - try sending him back as the escort. (I hear it has a high success rate)

Are you assigning everyone to the right positions? Are you sending Mordin back to the Normandy as an escort? He's squishy, so he's got a high chance of dying in the hold the line part.

For optimal Mordin survival, besides the obvious Escort spot, there is also Final Battle position though that could result in another squishy death if your HtL team is poor.
Best way of getting everyone out alive with everyone loyal, make sure you have at least 2 from Grunt, Garrus or Zaeed on the door defence team. That will ensure all your 4 squishies (Mordin, Tali, Jack and Kasumi) will survive.
