I was going to finally try to finish the game on Insanity for the achievement, but I have a quick question first. Does anyone know exactly how much of the game you have to complete in order to get the achievement? I was hoping I can just finish the major missions (like Noveria and Virmire) and not have to worry about all the minor stuff. I would really hate to find out I have to do it all over again.

wiki seems to say if the difficulty is on Insanity just before the end of eden prime and not adjusted afterwards, then you get the achievement.

If you set it to insanity before you touch the beacon on Eden Prime(don't touch it afterword) and do just the storyline worlds, that should do it.

I knew about setting the difficulty before leaving Eden Prime, but I just wasn't sure if I would be required to complete a certain percentage of the game or not. Thanks for the help.
