Despite better alternatives being available for use?  I know every other person is going to say "Oh yeah, the Cousland Family Sword! Or, at least, I take it out to stain it with Howe's blood". :P

Surana (Arcane Warrior, of course) will typically use Yusaris because of how you obtain it in the place he's spent the majority of his life.  It also has a very interesting history which he appreciates.

Tabris will typically get Topsider's Honor & Fang (two handed all the way for rouges ~_^).  Fang is self explanatory, but she keeps Topsider's Honor because she's totally brotastic with Oghren.

As for other characters, Sten of course keeps Asala.  I'll toss Alistair Duncan's Sword once I've got a hold of it.  Loghain gets Maric's blade, though. Leliana gets her mentor's bow.

Unfortunately Zevran & Oghren really don't have any specific weapon to hold onto and I doubt Wynne and Morrigan care very much about which staff they're using.

I dont use that much weapons or armors for sentimental reasons. Some yes but not all.

I usually give Alistair Cailans armor, shield and marics blade for the final battle. It fits because I make him king and he is about to take his fathers place. A king deserves royal equipment.

I also give Leliana Marjolaine´s bow.

Morrigan gets the robes of possession and Oghren being dwarf gets the legion of the dead armor.

To my human noble warden I give the warden commander armor because he is the leader, it has some nice advantages and it looks cool on him.

Otherwise I just give them what is usefull, although I often end up equipping them with the same equipment everytime. I dont know why, it just happens and they tend to look pretty cool with their equipment and are a destructive force with them.

Not a sentimental reason, but I inadvertently went through most of the alienage quests once with Alistair using the standard issue sword from Fort Drakon.

Cousland always uses the Cousland Family Sword to storm Howe's estate and kill him. Any of my sword-wielding elves take Topsider's Honor, since an elf used it. City elves always hang on to Fang, and now that I've picked up the mod that fixes it, any future city elf Wardens will actually equip it.

Morrigan stays in her original robes until she gets the Robes of Possession. Alistair ends up with Calian's armor and Maric's Blade if he's going to be king, and the Warden-Commander armor or Wade's Dragonbone Plate if he isn't. He always gets Duncan's Shield, and sometimes Duncan's Sword depending on the PC's class.

Leliana usually gets Marjolaine's bow. Sten keeps Asala. Wynne stays in her Senior Enchanter ones or the green one from Witch Hunt.

My Mahariel rogue only used Dalish armor and weapons, which meant her endgame armor was plain Tier 9 leather from Varathorn. But there ain't nothing wrong with Andruil's Blessing, and I realized that the Veshaille was Dalish in Redcliffe just before the armies marched. Almost couldn't afford it at that point but squeaked it out. :)

My Warden tends to wield Starfang and wear his suit of Wade's Superior <Armor of Choice>, except at the Landsmeet where I put Alistair in the full RTO royal outfit and dress the Warden up in the Warden-Commander armor.

I don't know about using, but I always keep a select few items, like the Cousland Sword and Highever Shield, which is weird when my canon Cousland is a rogue. Also, I don't know why, but my Rogue Cousland has always ended up wearing Wade's Superior Dragonskin Armor, after I talked to Dairren in the Origin, and said that his favorite book was a book about Dragons.

Alistair, on the other hand, tends to wear armor that fits him in the plot. As soon as I can get it, he wears the Warden-Commander armor. However, after he becomes King in the Landsmeet (if he does become King, anyway), he wears the full Cailan armor/arms set. If he stays a Grey Warden, he fights with Duncan/s Sword/Shield.

Leliana equips Marjolaine's bow, but that's supposed to be one of the best bows, so I don't know if that counts.

My (2H) Cousland used other weapons, (Starfang and Ageless in the end) but he always kept the Cousland sword and Highever shield in his inventory. Not the storage chest, mind you, the INVENTORY!

Last time I saw it, Cailan's Armor was worn by Oghren, which for some reason I found funny...

Pretty much all my characters use at least one origin related item throughout the game, but not always weapons..

CE: Adaia's Boots, Fang, and Wedding Ring (if applicable (and it has no stats on it!))
DE: Keeper's Ring
Mage: Ring of Study
HN: Family Sword (Curse my compulsive RP, that sword's terrible!)
DN: Aeducan Family Shield (Luckily, my only DN so far has been S&S)
DC: I haven't gotten a DC past Ostagar yet.. I imagine I'd use the Aeducan Mace though.

Leliana gets Marjolaine's Recurve; Alistair gets Duncan's Shield and Sword (If I'm playing a DW rogue I'll keep Duncan's Dagger). Also, Morrigan's Ring is a definite equip if I'm romancing her (I wish Leliana had something comparable..). Everything else is just decided by stats.

Yes, for both RP reasons and to try to increase the challenge of the game.

I keep items with sentimental value for my character, like the Dalish Elf's father's necklace and the Dwarf Noble's Aeducan Family Shield, or Sten's Asala, but my min-maxer nature means that I can't tolerate using them in battle. XD So I just put them into my storage chest, and have them use better gear in battle.

My Warden carries the Family Sword and Shield of Highever throughout Origins. Awakening he starts off with the starter Warden sword and shield, both of which get replaced by Vigilance and the Heartwood Shield. Vigilance eventually goes to Ariane, when my Warden used the Warden's companion once again. Morrigan's Ring is also in possession.

Morrigan wears Wade's Dragonbone Plate Armor, and the Cousland Guard Shield.

Leliana with the Warden-Commander Armor Set, including the accompanying sword and shield in the alternate slot (Marjolaine's Recurve becomes her primary weapon).

Sten wields Asala.

Sigrun wears the LOTD armor found in Origins (while Oghren wears his prepackaged one).

Nathaniel wears the Howe Shield found in Origins in the alternate slot (bow takes primary).

My companions get weapons that I think will fit my playstyle and thus their training.
Why let Sten run around with a tier 6 Sword when I have a lovely tier 7 weapon for him?
Same goes for Alistair, sure he gets the Cailan armor set, but not the sword because I take Maric's Sword combined with Starfang. The others mostly get whatever junk I seem to be collecting from time to time.
