For some reason it's fine now, but the Fade part of the Circle quest was really messed up.
Long waits for the game to realize that combat is over.
After combat is over and everyone puts their weapons away it still takes a while for an NPC to initiate dialogue. When you try to initiate dialogue with them to get it started, the conversation before the fight takes place and this is all that happens any time you try to initiate dialogue.
Takes a while for loot to show up on bodies.
sometimes the voice of the dialogue will be low and in the background, like I've initiated dialogue with someone off mic but still close enough for it to pick them up. This mostly happens to unimportant NPCs where the dialogue just appears above their head if you have subtitles enabled.
Had a conversation with Alistair where after choosing a dialogue option, skipping through it, choosing another one that I know led to a different result, he repeated the first response to the first dialogue option even though his lips were out of sync and the subtitles were saying the response to the second dialogue option I picked.
In my first playthrough I made it to the part where you needed to go to Redcliffe for the finale. The game pretty much told you, end game here! When I tried to go there I got an awful, almost game ending glitch. It would not load Redcliffe. Whenever I tried to travel there it would pop me back to the main menu of the game and say it couldn't load the area. I tried it several times from the same location and different locations and it would keep on doing that. I ended up shutting off the console and reloading the game and that worked.

This might be a crazy question, but how many people reporting bugs are actually playing the game while connected to Xbox Live? I have seen at least half of the sequences detailed above, playing as a Dwarf Noble (who never lost his dlc items after the Origin) but have never had a single thing bug out on me. I just moved and currently am not connected to the internet, so after looking through all the bugs here; (aside from Dex/Dagger issues) I am really starting to believe it is a Xbox Live/Dashboard running in the background problem. Oblivion was rife with issues while being played with your 360 connected to the internet, until they finally patched it. Loading times became insanely long, you would have to manually clear the cache using the old 'cheat' code or holding the A button down when you rebooted your console. Anyhow, I don't have an option to plug into the internet to test this theory by replaying different parts of the game to see if I can repeat any issues. I am half inclined to suggest that the problems most people are having seem to be the lack of flags registering, and I suspect the flags are being sent online to change your character status. Due to problems with the flags being set, it is causing a loop effect with the game state locally on the 360... hrmm... or something like that. I'm no systems analyst.
Knowing EA and their long-standing "first run" issues with their MMO's - maybe the internet code is jacked up... is there a way to post a poll to see how many have had bugs occur while connected to Live?

Hmm, just ran into a problem with Ser Landry duel nuking my party. All of their assigned abilities were empty and Wynn went to four tactic slots with four points spent on combat tactics. Going to see if reloading without Live running will fix it. If not I might revert to a previous save and see if things progress better without Live running. This error has repeated itself three times now whilst connected.

SPOILER...
So I ran into this glitch...
I go to the Landsmeet supporting Anora. I defeat Logain in an honor duel and let Alistair kill him. And instead of giving me the dialogue to show who I support for King/Queen, the game automatically decides I picked Alistair for King. A reload of a prior save does not fix things.
Another one I ran into was at the Proving. I would have to click on the 'proving master' icon off in the distance to trigger his 'ready to start the fight' dialogue AFTER I had already won the fight. I'd tell him 'not ready to fight yet' and it would then skip to the ending scene for the fight I just had and then sending me back to the Proving Master to start the next fight.

The F*cking redcliffe battle quest, i've killed all the corpses coming down first, then the messenger comes to tell me that they're attacking from the River.. guess what I kill all and nothing happens, so I go back to the hill, and i get an endless goddamn spawn of corpses.
