I lost Garrus, Tali, Zaeed and Mordin on my canon run. That was before I knew all the mechanics involved with making the choices. Never intended to have anyone die, but this was a suicide mission after all.

I have four hero runs, and the rest have variations of casualties. I have two characters where no one or very few went in loyal and I survived.

What I do now outside the hero runs, is decide at random how I make the decision to task teammates with the given objective. I roll a dice for (1 or 4: Favourable, 2 or 5: Non-Favourable, 3 or 6: Random). Also I decide by coin flip what to do with upgrading the ship, and what to do with Legion.

Do I ever kill anyone deliberately? No. By using randomness, I mix things up and that way I never know who is or isn't going to survive the mission.

No, but soon I'm going to start a run through from ME1 and pick all the 'wrong' choices so the Reapers win/come extremely close to winning. So hopefully I'll kill off all except maybe 3-5 of my squadmates. Just for the shiggles.

in a word, no.       i have multiple sheps, no one dies no one gets left behind.    the only dead squaddie in my play thrus  is the one i left behind on virmire.          oh and wrex(my renegade shep always kills that turd)

Personally, no. But someone I know killed Tali on every playthrough after a certain point, because he said every time he looked at her he thought about her fans and it ruined the character for him. Yeah, he'd had some run-ins with the more fanatical, less nice Talimancers.

I never killed any squadmates. In war, you need as many allies as possible and as many guns ready to fire.

Though as much as I hate Jack, I never killed her.

My cannon had everyone survive, my alts I intentionally killed Jacob (didn't do his loyalty mission, then sent him as an escort.)

At this point I have everyone alive...though I am still tossing up and thinking about killing Miranda off, I'm not sure yet...I will probably play ME3 the first time with everyone alive, but if I can't get Miranda off my ship in ME3...if she is an enforced major squad mate... I will probably go back to ME2 then and kill her off then see how that changes ME3....though I am still holding out and hoping we can all choose our major squad mates in ME3.

My canon playthrough was interesting. I had all of my squadmembers make it out alive, but my entire crew died. I wanted to be absolutely ready for the suicide mission, so I roamed around, doing side quests and visiting every hub to make sure I had everything. I didnt realize I was dooming my crew, and I was stunned when kelly became a smoothie. I had grown attached to them, so my canon shepard is abit depressing (Not to mention ugly, I gave him the worst complexion, hair, and maximized his Cheek Gaunt meter so he has really sunken in cheeks and a really long face. Ill be re working him in mass Effect 3. ). I have around 9 alts where everyone makes it out.
None of them have half crew, since Ken with no Gabby is so damn sad.
