After all she isn't confirmed as a playable character, it's logical to assume after the events of ME2 when the Alliance has 'reclaimed' the Normandy that Miranda and Jacob would of been taken prisoner as they are as far as the Alliance cares Cerberus Operatives.
Of course Cerberus can't have that so they bust them out and take them hostage or willingly perhaps. After all Miranda was a Cerberus loyalist only until the end of the Collector Base when she tells TIM to get bent.
Even if she isn't held against her will and having her DNA extracted by Cerberus; Miranda was genetically tailored in the first place as was her sister and I doubt it would of been that hard for Cerberus to get hold of the information to do it themselves.
Of course if she has been taken prisoner by Cerberus and she is your LI in ME2 perhaps there is a mission where you go to liberate her.

Cerberus has 150 OPERATIVES. Operatives are people like Jacob and Miranda, not just everyone in Cerberus.
And Cerberus didn't have 150 operatives after Retribution, it had even less. Cerberus loses and hires people all the time. If they didn't do that, the organization should be practically dead if you did the ME1 sidemissions.
40% sounds good, considering that unlike mercs, there is an actual plot point behind them.

Also Miranda said they have divisions( which is in military terms 10k to 15k soldiers).
So yeah, operatives aren't everyone in Cerberus.
