I have only ever played on Insanity (as I do with all RPGs, i find them too easy otherwise) The jax fight is darn near impossible at low level. The enemies simply regen to much life. It's best to complete that quest the easy way.
The other hard battle that ALWAYS kills 7-15 times are the two Turian assassin's outside fists bar... I hate those guys.

The room with the three Krogan gang and the Geth frog boys on Virmie as an fresh Engineer on Insanity is the hardest freakin fight in ME history. Took me darn near 50 tries beat them. If it wasn't for overpowered Liara I would've NEVER got through that. Urrgghh!
