I am having trouble playing on insanity with a new character. Do I have to start Insanity with a char that already beat the game before so I can play on new game + or something?
Tips for insanity?

New game + (insanity) is a lot harder than plain Insanity.
The game calculates the enemy's difficulty by the Level your Character is. This difficulty is flat regardless of upgrades ; Meaning that a NG+ LOKI mech is as lethal as what you'd face with a fully upgraded level 30 Sheppard. And this is the kicker; Redundant shielding , shotgun piercing ammo , additional damage for sniper head-shot , and so on , are not yet acquired when you start your NG+. So the first few missions are significantly harder even with max power levels ;Which are , in the end , mostly irrelevant in comparison.
General tip for Insanity; I struggled much , until i got through Hardcore first. I used to think Insanity was impossible , now i could do it in my sleep.
Specifically for NG+ ; One word ; Sentinel. Probably my favourite run was a NG+ Insanity Sentinel , the added difficulty really made the combat more Tactical/Thought provoking than anything I've played since.

You can do it NG or NG+.
NG+ insanity is generally a little tougher in the beginning because you and the enemies will be at a high (most likely max) level, but upgrades don't carry across. However the nice thing is you are able to play all the early part of the game with a complete power set.

I suppose i should correct my earlier statement; For certain classes , Power levels aren't as important as Weapons armour upgrades. In many ways its a reversal of a basic run through: Power users , like Adepts or Sentinels , are considered at a disadvantage until the Collector vessel. On a NG+ however they have the edge , since the extra weapon is already selected and Crowd control powers are equally effective regardless of enemy level; If you Throw an enemy off a ledge it will die whether its 30 or 10, for instance.

ME1 NG+ is much easier.
ME2 NG is easier.
So it will depend on which you are playing.
