Well if there could be some sort of reward in ME3 for that, then I'd say have something that has no effect on the game whatsoever, but is visible enough to mean something.
Maybe something as simple having the original N7 armor scheme from ME1 in ME3.
Below is the Heavy Version.

The game wouldn't have been half as fun as it is nor would the play-time value have been so long without Insanity.
Insanity offers one of the best singleplayer gameplay experiences I've had in video games. If you don't like it don't play it. I really can't see how offering more rewards would work without turning the game into a cakewalk.
Someone is too used to scrub shooters.
Insanity only longer simply because enemies had more health bars.

I just hope that the tougher difficulties are a little more balanced so that you can use whatever class and not worried about unfairly being destroyed. I don't want to feel like I can't use my Vanguard on Insane because she would be completely useless because everything has a shield and does ridiculous amounts of damage that almost makes close-range combat nearly impossible. I think that adding the roll for ME3 is at least a step towards the right direction.
Vanguard is definitely not a weak class on Insanity, but charge is a skill that requires practise. It's especially important to know when not to charge. Oh, and shotguns absolutely rape shields.

helps alot though once you master the vanguard on insanity it pretty much turns the game into a cakewalk

ME2 Insanity is really enjoyable.
No, it's not. It's one of the most frustrating hardest difficulties I ever played. Top two would be Ninja Gaiden II and God of War 2 though.
I spent hours trying to beat the final boss, hours trying to get past the Collector Ship and then Horizon, hours to get past Jack's mission...
The most messed up thing of all: Defeating the Shadow Broker was the easiest/most fun I had the entire run. Well, besides figuring out how to activate all 8 switches without really fighting, Vanguard's Biotic Charge made sure of that. I also had more fun going through Insanity in the first game.
Oh yeah, and I played it from a brand new game.

ME1 insanity was pure tedium mode it wasnt hard and what made it tedious was enemies spammed immunity all the damn time and you was practically unkillable to boot as well
and I seriously have no idea how it took you hours to beat individual missions because insanity mode is NOT that hard expecially if you abuse the crap out of squad mate skills yourself (and combo them like using overload/incinerate then follow up with a cyroblast instantly)) and dont charge around like a moron
I hard more trouble in the freaking prologue of the witcher 2 than I *ever* did in ME2

I'm 100% with you on this.. And im a gamer that usually plays every game on Normal. This the only game I have ever bothered to play on the hardest difficulty and i don't regret it!

Nightmare on DA2 was total and utter ****.
Brutal on SC2 was fun.
Very satisfying, and makes every victory, no matter how small the battle, mean something.

Just started a new Insanity run yesterday and I was so pissed about the pyro batarian waiting behind the corner when you have to seal the 3 shutter doors in Garrus recruitment mission that I turned down to normal just to get this slimebag out of the way.
All in all I don't think a reward is necessary, The biggest challenge on Insanity is being patient, there are few spots in the game were that isn't enough (I still didn't beat the Arrival Hardcore part on Insanity where the enemies just keep spawning and I think I tried it at least 15 times with three different chars, thanks for the minute long video sequence that you can't skip just before that bioware, I seriously hate that one, especially if you have to see it 5 times in a row).
Whoever managed that deserves a reward, although I would have tried it more often it there wasn't this goddamn video sequence Oo.
And in ME1 it doesn't work for me to even unlock insanity, because the game doesn't count it if I finish it on Hardcore Oo

I dunno, something neat like a little badge you get in the normandy cabin, or an award on the wall or something wouldn't be bad. Or some sort of unique paint job for your armor, just so when you show people your game they immediately know that "Oh damn, this guy's playing on Insanity!"
You would have to either explain it to that person or that person has to have played insanity to know it, so... not so cool?

I don't think so. I mean the internet exists, so we'd all see screenshots of what the "Insanity Insignia" or whatever is, so we'd know what it looks like. And lots of people put up videos of their games online on Youtube (there are a ton of vids for ME2 up there) and so on and so forth.

Same here, ever sine i unlocked it last december.
My avatar is very mis-matched, though on purpose. Got an ODST helmet, COG lancer, and N7 armor.
Just thought i'd put that out there for no reason.
