It only has to apply to someone that you COULD have killed, but didn't.
The events of ME1 happened, whether you played it or not. So even if you aren't an importer, that person would show up if they were certain to survive. If you never imported your ME1 Shepard, Wrex died. If you DID import, he had the chance to survive.
And Wrex's survival isn't a moral decision. It's not about Renegade or Paragon. It's about whether you had the speech points to do it. Or if you had his loyalty. I just feel like Wrex is the only one who fits this whole thing. I'm thinking maybe he gets Indoctrinated. I don't see him realistically turning on Shepard because of one of his decisions. But, he did it in ME1, so anything is possible. We didn't expect Cerberus to go full out kill squad on us. And that happened. I'm not gonna underestimate Bioware on any crazy plot points they whip out.

Only suggested it because the VI claimed she was the only person detected up on the platform AFTER the Rachni Queen encounter.
Also...she didn't see the light...and they always did tell her there would be a light.

Oh no, no, i get it now, it's after Shepards massacre of the Batarians. The wording made it seem like Balak had comitted a massacre himself, after the events of arrival.

Fist or helena blake even if they're alive you don't need to see them in me2 so again i think that rules them out, since in some playthroughs they'll be dead and in others you may not have even talked to them again.
Shiala again can't see it, me2 more seemed like you made the right choice for them to suddenly go no you didn't in me3 would be stupid i think.
Rachni queen again could be but its too obvious, every single renegede expects the rachni to turn on the paragons so if they go that route it'd be expected, i just think they'll go a different way with it.
The two most likely are Rana simply because of how your teamates react to you letting her live.
Or Balak especially with what happens in Arrival.
If i was to put money on it i'd say its Rana.
