Jeez, so many possibilities. I think having the Rachni completely betray you would be a bit over the top though, on the other hand though considering they were previously possibly indoctrinated by the Reapers perhaps they can easily be reindoctrinated?

Can't believe it is Toombs, he could die by my hand or shoot himself, so in the encounters there would be a 1/3 only that he lived.
And to take a guess I would say it would be someone most would recognise from a marked quest, IE. met in the main story line or DLC - so perhaps Balak, as an indoctrinated with "free" will akin to Saren, afterall we all know by now that Batarian space isn't the most funny place to be (not that it wasn't before for anyone else but Batarians, goddamn terrorists I say)

If you didn't have Wrex in your party during the mission (which I think is a bad idea to begin with), then no, Fist survives and pops up on Omega in ME2.

Very likely I'd say given that Fist is your typical criminal. He also worked for Saren, which means he does have some contact with the Reapers.

Not that I don't want there to be consequences for Paragon actions (that's a whole different discussion), but I would be disappointed if the Rachni queen turned on us. The Rachni as they are represent something very interesting to me: A completely inhuman, physically monstrous alien race that occupies a non-evil moral ground. I'd hate to see that spoiled, even by something like indoctrination.

Not sure if it's been mentioned but the creepy prison inmate that's carving your name in their victims is what came to mind. If not I'm still curious to see what happens with this guy and if it's going to be like the DA2 murder mystery quest.
