I started up another playthrough last night and I could no longer give Hawke a first name, thinking it was just a momentary occurence I continued playing. Later, when I went to edit the Dog's name it went through the cinema scene in Gamlen's home then jumped right to end, is this something Bioware decided to nix or just a glitch?

This happened several times with me as well. Not being able to give Hawke a customized first name, that is.
I don't know if it's connected, or just a random occurence, but as soon as the character customization screen pops up, if you head straight to the name selection, it works just fine. The problem only occured with me when I headed to the Import selection first, and then tried to customize the name.

Nevermind. It happened when I went straight to the name, too.

This just happened to me today as well. Any idea what is going on?

The Hawke name happened to me, I think it has to do with having too many existing games. Once u hit your limit, all future Hawkes will be named only by the default. Unlike Origins where u were just straight screwed and couldn't start a new game without deleting saves. I deleted all my abandoned and old ones and viola, I was able to give Hawke a name again. I haven't filled up again to test it to say for sure though.

So is there a limit on "dog's" name as well? Cause I just started a new playthrough and I was able to name my new Hawk but was unable to name his dog after the cut scene.

edit: so I moved or deleted old save files and then loaded the game then boom! Lame that there's a limit on dog's name too. smh
