Idea: ME3 should introduce paranormal/supernatural elements.

For example, in the midst of a firefight, a mysterious psychic force comes out of nowhere and kills the Cerberus troops you just engaged. Or, when wandering through an empty building, ghostly apparitions appear, startling you and your squadmates.
It could make for a nice subplot if you think about it. For example, Eden Prime exhibits unnatural instability far beyond the norm when Shepard lands there, and he/she discovers that a civilian killed during the Reaper's attack on Eden Prime is the reason for the hostile spectres present on the planet, making it dangerous for anyone to land and threatening to force the planet off its orbit. To restore stability, Shepard must properly lay her soul to rest.

I wouldn't mind Overlord-esque visual hacks though, but this just doesn't fit in the ME verse.

Imagine Shepard getting a chance to talk to Ashley/Kaidan in the afterlife if he/she sacrificed him/her on Virmire. It would be awesome.

Introducing paranormal elements into what thus far has been mostly straight sci-fi would be the height of silliness. If I want to play F.E.A.R. or Silent Hill, I'll go do that because there are existing games that revolve around the paranormal and psychological horror. *sigh*

Though I would be tickled if it turned out Shepard died for real, and Lazarus Shepard were really an Angel sent from God, disappearing at the end of ME3 after his mission on the mortal realm is done !!!

Supernatural things should only happen in dream-like states (or when Shepard is under some form of mind-manipulation or whatever).

The "soul" does not exist. There is no evidence that even remotely alludes to it's presence. by extension, this rules out the existence of ghosts, too. Or any other psychobabble bull****.
Everything in the Mass Effect universe established so far is scientifically plausible. Not "possible," mind you, but "plausible."
