In my import save, I had to keep everyone alive for the achievement.
First game turned into just wanting to beat it to see the ending. Second game was played on Insanity so I only went through what I needed to and Shepard died because everyone but Morinth died.
So, yes, the third game everyone I recruited had to be alive and I wasn't going to beat the game again without experiencing everything.
If I were to play again and killed off people or just didn't recruit it would go like this:

Thane
Grunt
Mordin
Zaeed
Jacob
Legion
Survivors:
Morinth
Kasumi
Miranda
Tali
Jack

Idk, I mean, I just completed my first playthroughs of the games. In ME1 only Kaidan died, then in ME2 Thane & Jack died (all the crew lived).
I actually liked that outcome despite loving Thane as a character in general (one of my favs) and at least loving Jack in combat (otherwise she kind of annoys me, but she does make me laugh sometimes). Reason being, with going on such a big 'suicide mission', I think it's realistic that not everyone will survive, and when Thane died it was such a shock to me and I was all NOOO!!! But afterwards I realized that was a cool genuine reaction and that's part of the fun of the series, not knowing exactly what will happen, be it because of your decisions or whatever.
Granted, I did all the loyalty missions, all the side missions, got all the ship upgrades and whatnot, and heard they had a outcome on what would happen, so I didn't really expect anyone to die. So when they did I was shocked and kind of glad (not in a sadistic way lol, just cool from a storytelling perspective and the fact I legit didn't expect it).
What happened was I had Jack as my biotic barrier person and I lost her loyalty in the Miranda argument and couldn't win it back, so Thane got taken by Seeker swarms because of it, then Jack herself died on the final fall because again, she wasn't loyal. (lol she was the cause of both of my deaths!)
So I liked that outcome, I lost one of my favs (Thane), so it made me really care about it, and while I didn't particularly love Jack as a character I thought she was awesome in battle like I said. (Granted if I lost Garrus or Mordin I might've had to restart lmao). Whenever I do my 'final' playthrough before ME3, I should be able to avoid any deaths, but Idk if I want to just because a couple impactful ones can be good from a storytelling perspective.

Ditto here....never lost anyone in my 12+ playthroughs.
And I'd rather not leave anyone behind unless I have to (read: forced to) as well.

There are some walkthroughs and the Wiki that can help you with that. But unless you choose one specific person I think you will always loose someone at the final door. At least I haven't found anything that inidcated otherwise.

Sometimes people will randomly die holding the line. I think Mordin is one of the more common ones that will die for apparently no reason, so it's a good idea to always take him with you for the final battle.
Mordin is the squishiest and the Doc. Better to send him back to the ship with the traumatized crew. A no brainer really.

Why not mix it up some instead of having everyone in every playthrough? I like variety. Which is why now, I am actually letting people die in the SM, so I can see the difference in ME2.Merlin 47 wrote...
