I'm aiming to get all achievements (Xbox 360) but not really looking forward to this one, as I normally play on Casual or Normal difficulty. I did manage the Varterral on Hard with no problems though.
So how hard is "Grim Reaper" really?

Personally, I don't think this trophy (I'm on the PS3) was hard at all. In fact, I replayed GoA on nightmare difficulty a couple of days ago without much problem; I mostly controlled my warden and let the 2 dwarfs follow the preset tactics. However, this can change considerably depending on your character's build, level & equipment. 
I used a level 35 Rogue (Assassin/Duelist/Shadow) with points in both DW and Archery (50/50 Dex/Cun) & switched between the weapon styles depending on the situation. There is a room with 6 golems (4 Boss, 2 Elite) that was a little though, but that room is not even necessary to clear to complete the DLC. The end boss itself was actually pretty easy, just make sure to kill of all of the lesser enemies that spawns during the battle.
If you import a character that has completed most side quests within Origins and Awakening and also has powerful items like Lifegiver and a good set of armor you should be able to complete it on hard without much trouble.
PS.
Buying some Manuals of focus in Awakening for the 2 party members in the GoA is recommended.

You can play golems on any difficulty as long as you change it to hard before the harvester fight.

The harvester fight is tough, in that you not only fight the harvester, but several undead as well, who can range anywhere from normal to boss rank (rank is random). They can take a well built tank down in a hurry if you're unlucky.
While I don't have much trouble with anything from Origins to Witch Hunt, this fight usually has me reloading a few times per run, unless I get lucky and the undead are all low rank. It is also highly recommended to import a level 35 warden from Awakening with not only at least two manuals of focus to respec your dwarven allies, but bring as many health poultices as possible and the best gear you have on hand, such as the sentinel armor for your warrior ally and a good bow for your rogue ally.
I actually have less trouble with the golems in the green room. The harvester fight can be easy or tough depending on the rank of the undead that fight alongside the harvester. If they're all or mostly boss rank, then it takes time to kill them, and the runic golem isn't much of a healer. The switch in the room isn't always available either (usually when I really need it). Without a true mage in the group (unless the warden is a mage), your options are limited with crowd control and healing.
Just try to be patient and learn from your mistakes if you find yourself reloading often.
As for the two dwarven allies, I usually respec Jerrik to an archer who uses accuracy+rapid shot and make him a bard/ranger (you're stuck with ranger as a spec for him). Brogan makes a nice tank. Usually I go with champion/guardian and try to get his dexterity as high as possible, while getting strength around 52 for talent/gear requirement, and some points into constitution.

The hardest part is shaping up your temporary companions into something approaching a useful state. Every single one of them are an embarassment in combat.

Agreed. I can't imagine doing this with a new character, created for GoA.
This dlc would have been so much easier with a real healer instead of the runic golem. Playing Witch Hunt after this feels like a breeze having Finn on hand, as well as no golems or harvesters there.

Max level rogue archer using Awakening classes and bard abilities can hit the Harvester for around 250-300 damage per shot on the Hard difficulty. Traps can also seriously cripple the undead, especially gravity traps, placing them prior to the fight is entirely possible if you know where the trigger for the cutscene is located. Also Demonslayer (+20 damage vs. undead as an item property, with grandmaster cold iron runes and paragon fire runes that are boosted by the runic golems flame weapon spell), imported from Awakenings and given to a respecced 2-handed Brogan will quickly kill the undead (though only 150ish damage per hit on the Harvester).
The fight is all about prep work and less about strategy, getting the respec books and Awakening gear unequipped in a save and ready to transfer over is the hardest part. Once you bring in max level characters and high level gear the fight is pretty easy with only a moderate amount of management on the players part.

As stated its best to prep for the dlc.
by which I mean have gear for your two companions in your inventory and respec manuals for them.
Tanking dosnt work so well in this encounter as much as dealing damage does. Make your dwarf a bard and the other a two hand warrior.
Of course your Wardens class can affect what you want your party members to do.
Though a Duel Wield or two hand Warden Warrior with 100% Spell Immune and 100% dodge has a easy time with this encounter.
