Templar Nightmare...HELP!

However, i've been running around for hours at templar nightmare area, in all forms, and i can't freakin find the boss demon on this level. I keep teleporting back to the same areas and all the mouse holes lead me back to the same areas as well..
can anyone help guide to where i need to go?

If I remember correctly, the boss demon for this area is behind a Massive Door to the north of where you ended the Templar's nightmare.

Off the top of my head, at least one mousehold leads to the next section (third of tower), and at least one invisible door does the same.

Given the huge number of permanent attribute point buffs scattered around in hidden parts of the dungeon, getting all the forms first is totally required anyway, unless you want to run a gimped character for the rest of the game. I think I counted about 17 or 18 free attribute point increase places scattered throughout the area, and many you have to back track to get to since you won't get the skill to access them until long after you pass it by. There are 4 Willpower increases alone scattered around. There are 2-3 point increases of all other attributes as well, so don't skip any part of the dungeon.

You usually acquire each form after you need it to reach some mysterious passage or door, so you end up backtracking a lot. To complete the Templar area you need all FOUR forms, Mouse, Spirit, Burning Man, and Stone Golem. I think you can get to the final boss on that level, Vereveel, by turning into a Golem and breaking down the door, or turning into a Mouse to take a mousehole.

Great suggestion, I even skipped out on going to the sloth demon right away and found some of the free att points that I had missed earlier. Golem form is amazing btw!
