What's your favorite?
For me there's some duos that work so well together that my Shep can't leave home without them. (always on Insanity so I can't say about lower diffs... should be so much easier that it isn't even fun)
Against Harbinger - Anyone plus Kasumi. Her grenades keep him absolutely inert. He never injured my Shep even once in many runs! Kasumi is a must have vs that thing on Insanity.
Husks - Grunt and Jack - With their own ammo powers and Shotguns! Plus Shep with Shotguns... I could go thru some side missions plus the IFF mission with them without a single death. Jack's shockwave can also throw/blow multiple Husks at once when their armor is down!
Geths - Miranda and Tali - Heavy Overload and Area Hack rules! - Works fine vs Mechs too.
Eclipse and Blue Suns - Zaeed plus anyone with Overload. I like Garrus or Miranda. Sometimes Mordin or Thane. I love making them scream when incinerated!
Blood Pack - Grunt. In a Krogan vs Krogan he's imbatible! Plus a number of possible companions.
The only companions I hardly use are Jacob and Samara.

Depends on what class Shepard is. If he's anything that's got soldier in it (IE Soldier, Vanguard, Infiltrator) then I won't have Garrus or Zaeed in my party. If I'm an Engineer, I like to have Tali and Legion in my party but... Since you get them so late in the game, I usually stick with Garrus and Jack until that time.
Garrus and Jack are usually my dream team regardless. It's quite rare that I'll go through the game without Jack. Sometimes I'll switch Garrus out (Even though I miss his personality) with Kasumi or Legion.

Against armored opponents such as husks, it depends which shotgun, only eviscerator and claymore have an armor bonus. Handguns, assault rifles, and sniper rifles have a bonus against armor. The Locust is the only SMG that has an armor bonus against armor.
Inferno Grenade, Reeve, Incinerate, Incendiary Ammo, Warp, and Warp Ammo have a bonus against armor.
So, against armor, If I already have Warp Ammo or Incendiary Ammo, then I'd take Zaeed, Samara, or Mordin. Although, it's tempting to take Grunt to watch him waddle into husks. Jacob could complete the pair as his little shield brother. Miranda will work too.
Against shielded enemies I take someone with overload or energy drain if I already have disruptor ammo. So, that would be Garrus, Miranda, Tali, or Katsumi. Usually, I take Garrus and Miranda because Garrus has a good power boost and good weapons and Miranda has warp which you'll probably find armor or barriers to use against since they are often present with shielded opponents. If I don't have disruptor ammo, then I take Zaeed, who has a huge damage bonus and good weapons too.

I used Miranda a lot in my first playthrough and then the next time I wondered if I'd missed some guns or upgrades because the damage bonus actually does make a noticeable difference quite often.
I had to do at least one playthrough with Tali/Garrus, who I also used pretty much exclusively in ME1 with the same character. Tali isn't as useful when not fighting synthetics/shielded enemies but I like to use the "wrong" characters in missions sometimes (and the Drone is often good anyway).
Mordin goes great in any team too, especially for the dialogue (though Incinerate is nice against the majority of enemies).
I never really learned to use Jacob, Jack or Grunt well in combat so I tend to leave them behind, I might try working them into a future playthrough.

On my rev soldier, I have grunt and legion. Grunt has the claymore, and legion has the widow.
And like I said, the soldier has her rev. Awesome times.
On both my engineers I do tali/legion and control the battlefield with our combat drones. 3 drones punking harbringer is awesome.

Poor Jacob is the least used by most players? heh I think I gave him some role on the Suicide Mission in one of my runs though, and he delivered! At least that. =)

I tend not to play on the harder difficulties because they just ****** me off. and on normal/veteran I can take whoever I want with me. This usually ends up being Tali/Jack, or Tali/Garrus for a while. Then Legion joins my squad and it's Tali/Legion from then on. Geth/Quarian tag team FTW!

Seriously though that is a surprisingly fantastic team. I ran them on Insanity and dominated the game with no effort whatsoever. Kasumi tends to frequent most of my other party setups though, with a special place for Miranda and Garrus. I like Jacob but he is just so damn outclassed. I have yet to put any real use out of him, Samara, Mordin, Jack, Tali or Legion. Just better options, and Legion joins far too late.

I try to like Jacob. Took him with Grunt to Horizon with my Infiltrator Assassin but he's useless! At least on Insanity. It was like my Shep was with Grunt only. At least they made it to the end of the mission alive... unlike poor soldier boy there.

I think part of the problem with Jacob is that he can only fill a couple of roles and there's at least one other teammate that is better for each role. For example, Jacob's pull ability may be useful but Jack goes one better by having Warp Ammo (which does extra damage to pulled targets). Jacob is reasonably tough and can fight close quarters with his Shotgun, Barrier and Incendiary Ammo but Grunt has basically the same and is much tougher. He also lacks any direct damage/defence stripping attacks, which are fairly popular.RageGT wrote...

close quarters : usually i will use grunt for a tank, with heavy armor and a strong shotty he is pretty buff. also kasumi is very helpful, because in closed areas she can easily cloak around wacking your enemies.
wide open (outside) missions : garrus for sniper always, then either zaeed to compliment him. or samara and thane for some long range + biotics.
collectors : tali for combat drone always, then either miranda or a weapon powerhouse. miranda is usefull for the abilities, but i like to compliment talis drone with another soldier (like me) such as garrus/zaeed to lite up harbinger when he is turned toward talis drone. usually for the human reaper tali will be subbed for legion, because of his drone + sniper.
i always like to have atleast one squaddie with warp, as that spell is terribly abbused when i play this game

I think I'm going to start a new game soon... This time I'm going to use characters I rarely use (But not Miranda. I just hate that ****)
Perhaps I'll forego Garrus and Jack completely (Though this makes me almost cry) and use... I dunno, Grunt and Jacob? Maybe Grunt and Morinth/Samara... I haven't done Adept in a while, maybe I'll do that with Grunt and Zaeed
