Do you mean in-character? Why does it need to be?
What's the in-game logic behind difficulty levels? What magic force suddenly makes that Blue Suns merc harder or easier to kill?

They could force us to watch cutscenes when we launch the game for the first time, and from then on they could allow us to go directly to the CC.

today people can change their haircut, their makeup, get a tattoo, change their nose, look younger, etc, etc...
200 hundreds years from now i'm pretty sure much more than that would be possible. what is it that can't see ?

In ME2 Shepard's put back together exactly as they were. Ironic considering how Shepard's face depending on options chosen changed from ME to ME2. In any case, what's said in ME2 is key. Shepard was brought back exactly as they were. Everyone else concocted a reason as to why Shepard's face changed if you made alterations when the only thing that possibly could have been altered is the scar on his/her face. Everything else is the same.

Only because I would use it through out the entire game. I´d rather have the face gen at the beginning of the game like in ME1, in a way it wouldn´t require restart everytime I use it.

That'd be ideal for me as well. If they felt the need to give it an ingame justification, then I imagine "Dr. Chakaws' Plastic Surgery Device" would do the trick. I personally do not think its' necessary but some do.
